The Resident Service Coordinator is responsible for ensuring that residents are linked to the supportive services they need to continue living independently in their current housing environment. Overview
The Service Coordinator plays a critical role to support HUD-assisted multifamily housing as a platform for financial security, physical security, social connections, and the delivery of long-term community based supportive services, especially for vulnerable populations. Service Coordinators help to coordinate the provision of supportive services to help residents continue to maintain their independence and delay or avoid the need for higher levels of care (e.g., an assistive living facility or skilled nursing facility). The Service Coordinator manages and provides access to necessary supportive services in the community, provides case management services as needed and requested, and develops programs and resources that support wellness for the entire resident population. ESSENTIAL CORE FUNCTIONS:
· Provides general service management which includes conducting non-clinical assessments of residents for wellness and social needs to develop individual supportive services management plans.
· Helps residents to identify, access, and coordinate services (such as personal care services), including monitoring of services provided and follow-up communication with service providers.
· Monitors the receipt and follow through of services. Encourages and motivates residents to engage with providers and participate in their own care/services management.
· Proactively develops and arranges wellness and other educational programs and services for residents.
· Develops and maintains an up-to-date resource directory with all local service providers.
· Actively creates and sustains new partnerships with community-based supportive service providers, and other community stakeholders.
· Utilize and manage volunteer engagement in coordination with the Director of Programs to expand programing to residents and utilize specific skill sets.
· Meet the minimum training requirements as defined by HUD, as well as maintain ongoing continuing education requirements.
· Demonstrate resident confidentiality and code of ethics and comply with Fair Housing Laws.
· Familiarity with and commitment to FHR’s Values and Behaviors, Mission, Vision and Diversity, Equity and Inclusion Commitment Statements.
· Coordinate newsletter content with Property Manager.
· Work within prescribed budgetary constraints.
· Participate in site staff and corporate meetings, inform and train other staff about the coordinator role and issues related to aging in place.
· Other duties outlined by HUD or assigned by the Director of Property Management.
EDUCATION AND EXPERIENCE:
·Possess a four (4) year college degree with a major in social services, or equivalent combination of education, training, and experience. One (1) year experience in related field; minimum of one (1) year experience in subsidized housing preferred
· Knowledge, training, and direct experience in elderly and disability services is preferred.
· Must be able to advocate, organize, and problem-solve.
· Strong working knowledge of local community services preferred.
· Self-motivated, keeping up on new policies and HUD regulations and implementing changes in relation to role.
· Ability to maintain strict confidentiality and follow policies and procedures.
· Must have excellent oral and written communication skills serving individuals from diverse backgrounds and experiences.
· Skilled in Microsoft Office 365, including Word, Excel, Power Point, SharePoint, and One-Drive.
· Bilingual in both Spanish and English.
OTHER REQUIREMENTS:
· Must have a valid Arizona Driver’s License.
· Must have a working vehicle with current tags and be able to provide proof of insurance as travel will be required.
· Ability to pass a company background check.
PHYSICAL AND VISUAL ACTIVITIES:
Ability to interpret, evaluate and communicate detailed written or verbal instructions to others accurately and quickly, including answering phones, attending meetings, written correspondence, or other forms of communication. Ability to lift, push, pull, or carry up to 15 lbs for the disseminations of documents, supplies, materials, etc. to various locations throughout the office/facility. Ability to communicate effectively (expressing or exchanging ideas by means of spoken or written word). This skill/ability is important in work activities in which workers must impart or receive detailed information through oral or written communication.

WORKING ENVIRONMENT:
· Office environment at residential property and some travel to various locations in Tucson and Southern Arizona.

Pay: $32,000.00 - $36,500.00 per year
