Who are you?
You are a professional that has any combination of education, training, and work experience which indicates possession of the skills, knowledge, and abilities needed to perform the listed job duties. An example of an acceptable qualification for this position is: Completion of accredited PNP program that includes clinical experiences in all aspects of behavioral healthcare including both mental health and substance use disorders across all age groups. 1 year of post degree experience is preferred. Must possess Ohio Licensure as a Nurse Practitioner, certification by the American Nurses Credentialing Center as a PNP, an active DEA License and X-Waiver. Background check and fingerprinting required.
You will possess skill and experience in assessment and treatment of patients with a behavioral health and co-occurring conditions including mood disorders, psychotic conditions, attention deficit and impulse control disorder, substance use conditions and eating disorders while working collaboratively with multi-disciplinary teams. You will demonstrate the ability to work with diverse populations as well as cultural competence. You will have knowledge of electronic medical record systems with prior telehealth experience being preferred.
How you’ll make a difference in this role:
You will practice within a Team Based Model ensuring integrated care that addresses behavioral health and primary care conditions through collaboration with all disciplines involved in the treatment of individuals served. Your primary responsibility is the provision of direct care services including review of health assessment forms, completion of comprehensive mental health evaluations, establishing behavioral health diagnoses, determining appropriate treatment plans, provide ongoing care and making appropriate referrals. While in this role you will also do the following:
- Engage with families and other support systems by providing them with resources and information about behavioral health conditions. You will assist them in understanding how they can be supportive of recovery efforts.
- Comply with treatment protocols established to address mental health and substance use conditions.
- Work collaboratively with staff and external partners to best serve the needs of individuals served.
- Complete documentation in a timely manner using E/M standards. Use of concurrent, collaborative documentation is strongly encouraged.
- Participate in peer review as part of the ongoing CQI process
- Complete CME as required to maintain licensure
- Develop skills and remain aware of new treatment methodologies while also supporting their implementation as guided by Medical Director.
- Maintains required direct service productivity level
