Department:  Assertive Community Treatment (ACT)  Job Status:  Full-Time                

FLSA Status:  Non-Exempt                                                       Reports to:  ACT Team Manager

Positions Supervised:  None                                                  

Amount of Travel Required:  90%; Shawnee county

Work Schedule:  Monday through Friday, Varied hours between 7 a.m. to 7 p.m.                 

Must be able and willing to adjust work schedules based on their clinical case load and the needs of individual clients and their families.

 

Position Summary

The Youth ACT program is designed to address the needs of clients ages 13-21 who are at risk of entering or are returning home from high intensity services such as inpatient settings or psychiatric residential treatment facilities. The ACT Case Manager will provide medically necessary services at the direction of a qualified mental health provider skilled in the treatment of mental disorders to assist the client and their families in ameliorating the effects of the mental and emotional impairment for which institutional level of care has been indicated. The ACT Case Manager provides goal-directed supports and solution-focused interventions intended to achieve identified goals and objectives as set forth in the client’s individual treatment plan. ACT Case Management is a face-to-face intervention with the client, guardians, and other treatment team and community members. The majority of ACT Case Management contacts must occur face-to-face and in community locations where the client lives, works, attends schools, and/or socializes. The services will be specifically for youth and their families enrolled in the ACT program with the goal to enhance independent and interpersonal functioning such that the client is integrated into and/or maintained within the home and community so that institutionalization is not as likely or frequent.

 

ESSENTIAL FUNCTIONS

  • Provide direct services to youth and their families who are enrolled in the ACT program.
  • Provide direct services based on the philosophies and principles of the strengths model. Assist the youth, family members, and other collaterals as appropriate with identifying strengths and needs, resources, natural supports. Assist in clarifying goals and objectives to utilize personal/familial strengths, resources, and natural supports to address functional deficits associated with their mental illness to reduce risk of institutional level of care.
  • Provide supportive counseling to the youth/family with evidenced-based interventions for problem behavior analysis and emotional and behavioral management with the goal of assisting the youth/family with developing and implementing social, interpersonal, self-care, daily living and independent living skills to restore stability and support functional gains.
  • Serve the duties of a peer support specialist to coach youth/families to promote recovery and self-direction.
  • Facilitate wellness management and recovery strategies.
  • Model skills for and provide consultation to youth, families, and fellow team members.
  • Assist in identifying and developing employment skills and opportunities.
  • Provide crisis services as needed for the client/family
  • Provide parent support services to assist in developing a positive strengths based environment in the home
  • Provide coordination of multiple services for youth and families.
  • Act as an advocate for children/youth and families. Collaborate with community, academic, and legal programs to increase functioning for the youth and their family.
  • Assist in tracking progress of the client and their family by utilizing objective, valid, and reliable measures at the direction of the ACT Team Leader or other qualified mental health professional.
  • Attend ACT Team Meetings and actively participate in the review of clients’ progress and services.
  • Complete required trainings.
  • Complete required background checks.
  • Complete documentation (progress notes, treatment plan reviews, etc.) in accordance to agency standards. ACT Case Managers are also expected to stay informed of agency developments by reading memorandums and other agency related written communication.
  • Respect and maintain client confidentiality. Report any breech to supervisor and Risk Manager.
  • Participate in all ACT team activities and consult with team members as an equal professional.
  • Provide cross training to other team members in recovery principles and strategies.
  • Act as an ambassador of Family Service and Guidance Center in the community setting.
  • Support and actively promote the vision and mission of Family Service and Guidance Center and the philosophy of the strengths perspective.
  • Other duties as assigned by respective team manager or supervisors.
  • Assist in maintaining and meeting fidelity standards of the ACT program

Qualifications


Education/Licensure:

  • BA/BS degree in a human service field or 2 years of education/experience in a human service field.
  • Must successfully pass the KBI, Child Abuse, Adult Abuse, Motor Vehicle Record and all Central Registry background investigations.
  • Valid driver’s license, current auto insurance, and a reliable vehicle are required

 

Experience:

  • Must have 1 years of experience working with children, adolescents, and/or families
  • Experience working with youth identified with severe emotional disturbance (SED) preferred
  • Lived experience preferred

Other Physical Requirements:

ACT Case Managers/Peer Specialists must have sufficient ambulatory capabilities to work with children, adolescents, young adults and their families in the home and community based setting. Position may require the ability to use physical holding (MAB) with children and adolescents exhibiting out of control behaviors.

 

WORK ENVIRONMENT

Office and community-based working environment; requires flexible schedule; frequent interruptions occur; expected to deal with individuals and families in crisis; risk of exposure to verbally and physically aggressive youth; and exposure to low socioeconomic homes and neighborhoods.
