Job Title: General Surgery Physicians
Job Summary: FAR Group purpose of this contract is to acquire GENERAL SURGERY PHYSICIAN SERVICES candidate(s) to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Veterans Healthcare System of the Ozarks (hereinafter referred to as VAMC) The physician’s care shall cover the range of General Surgery services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Board of Surgery (ABS).
Place of Performance: Services shall be provided on site, VAMC 1100 North College Ave. Fayetteville, AR. 72703
QUALIFICATIONS:
License: The physician(s) shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property.
All licenses shall be full and unrestricted licenses. Contractor’s physician(s) who have current, full, and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action, or denied upon application will not be considered for the purposes of this contract.
Board Certification: The physician shall be Board Certified or Board Eligible by the American Board of Surgeons and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must always be kept up to date.
Credentialing and Privileging: Credentialing and privileging is to be done in accordance with the provisions of VHA Handbook 1100.19 and VHA Directive 1100.20 referenced above.
Technical Proficiency: The physician shall be technically proficient in the skills necessary to fulfill the Government’s requirements, including the ability to speak, understand, read, and write English fluently.
DEA: Contractor shall provide a copy of current DEA certificate.
VA HOURS OF OPERATION/SCHEDULING
VA Business Hours: Daily business/clinic hours will be Monday-Friday 8:00 am - 4:00 pm. On call hours will be from Monday-Friday 4:00 pm -8:00 am. Weekend call will be from Friday 4:00 pm - Monday 8:00 am.
Clinical Personnel Required: The physician shall be competent, qualified per this performance work statement and adequately trained to perform assigned duties.
Standards of Care: The physician(s)’ care shall cover the range of GENERAL SURGERY services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized TJC, VA and national standards as established by The ABS.

Scope of Care: Physician (as appropriate and within scope of practice/privileging) shall be responsible for providing GENERAL SURGERY Physician care, including, but not limited to:
Clinic and Surgical Care: Physician(s) shall provide clinical GENERAL SURGERY physician services. Physician(s) shall be present on time for any scheduled clinics/surgeries as documented by physical presence in the clinic or operating room at the scheduled start time.
Operative Services: Physician(s) shall provide comprehensive clinical General Surgery Physician services, including but not limited to the following:
- 99202 : Office o/p new sf 15-29 min.
- 99203 : Office o/p new low 30-44 min.
- 99204: Office o/p new mod 45-59 min.
- 99205 : Office o/p new hi 60-74 min.
- 99211 : Off/op est may x req phy/qhp.
- 99212 : Office o/p est sf 10-19 min.
- 99213 : Office o/p est low 20-29 min.
- 99214 : Office o/p est mod 30-39 min.
- 99215 : Office o/p est hi 40-54 min.
- 44950 : Appendectomy.
- 44960 : Appendectomy w/ rupture.
- 45378 : Diagnostic colonoscopy.
- 47563 : Laparo cholecystectomy/graph.
- 49565 : Rerepair ventrl hern reduce.
- 49568 : Hernia repair w/mesh.
- 49505 : Prp i/hern init reduc >5 yr.
Intraoperative Follow-up: Physician(s) shall be present in the operating suite for all GENERAL SURGERY procedures.
Physician(s) rounds shall be conducted on postoperative patients in the Surgical Intensive Care Unit (SICU) and on the wards.
Physician(s) shall provide consultative services at the patient’s bedside if the patient is not ambulatory and, in the clinic, setting if the patient is able to report to the outpatient clinic. Procedures shall be scheduled for completion within 30 days of the date of the consultation.
Medications: Physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients.
Discharge education: Physician(s) shall provide discharge education and follow up instructions that are coordinated with the next care setting for all GENERAL SURGERY clinical or surgical patients.
COMMUNICATING TEST RESULTS TO PROVIDERS AND PATIENTS: In accordance with VHA Directive 1088, Communicating Test Results to Providers and Patients, all test results requiring action must be communicated by the ordering provider, or designee, to
patients no later than 7 calendar days from the date on which the results are available. For test results that require no action, results must be communicated by the ordering provider, or designee, to patients no later than 14 calendar days from the date on which the results are available.
ADMINISTRATIVE: Estimated 10% of time not involved in direct patient care for charting purposes.
Quality Improvement Meetings: Physician(s) shall participate in continuous quality improvement activities and meetings with committee participation as required by the VAMC Chief of Service, Chief of Staff, or designee.
Staff Meetings: Physician(s) shall attend staff meetings as required by the VAMC Chief of Service, Chief of Staff, or designee.
QA/QI documentation: Physician(s) shall complete the appropriate QM/PI documentation pertaining to all procedures, complications, and outcome of examinations.
Patient Safety Compliance and Reporting: Physician(s) shall follow all established patient safety and infection control standards of care. Physician(s) shall make every effort to prevent medication errors, falls, and patient injury caused by acts of commission or omission in the delivery of care. All events related to patient injury, medication errors, and other breeches of patient safety. shall be documented in the medical record of those impacted and disclosed to the patient or surrogate.
