Division Description
The Children, Youth, Family and Transitional Age Youth services Division (CYFTAY) offers an array of services to underserved clients, with multiple challenges and abilities including: Infants, toddlers, and preschool children, including those with special needs. Youth: including transitional age youth, pregnant| parenting teens and youth in the foster care system. Families: clients with limited access to financial resources, immigrant, monolingual, and homeless populations. The CYFTAY division offers multidisciplinary programs including Early Care and Education, Early Intervention| Inclusion, Family Support, Mental Health Services, Health, Nutrition, Wellness and Parenting Education Programming.

 

Position Overview

Reporting to the Administration Director, the Registered Nurse Care Coordinator is an expanded position directly connected to our strategic priorities where we will focus on the health and wellness of children, families and educators. As the pandemic, along with a variety of civil unrest have affected the routine and structure of our daily lives, deliberate attention to health and wellness is more important than ever. In addition, social determinants of health such as poverty, lack of access to healthy food etc... are rooted in structural racism that creates risk factors for poor health and wellness outcomes. Using a multi-sector, place-based and collective impact approach that acknowledges that children are touched by sectors across health, education and social services, Felton Institute will focus on these four areas: health, nutrition, physical activity, and emotional wellness. To reach this goal, the plan includes, strengthen internal capacity and infrastructure (internal work) to enhance and expand client centered, equity centered health and wellness initiatives across the CYFTAY division. Our main goal is to create healthy social, physical environments that improves the health and wellbeing for all by 1) Ensuring children are healthy 2) Reducing childhood obesity, 3) Improving culturally responsive and healthy food options at school and 4) Improving physical activity opportunities. This position will play a critical role in developing and implementing services and strategies that support these goals.

 

We are looking for a Registered Nurse Care Coordinator with deep domain expertise in the health and social services sector, who will lead our health and wellness programming across Felton’s ECE programs. The successful candidate will bring previous experience in a public health setting, ideally a school setting. S/he will be intellectually curious yet grounded in his/her awareness of others’ perspectives and sensitive to the challenges that CYFTAY clients and direct-service staff experience. This is an exciting opportunity for a thoughtful, hands-on healthcare advocate to have a significant impact at Felton’s CYFTAY division.

We’re looking for a caring, compassionate, experienced professional who loves taking care of children and prioritizes communication, engagement and a problem-solving mindset. The position requires administrative work as well as one-on-one time with children. The ideal candidate will support to help bridge health care and early care and education to facilitate optimal development and academic success. He/she will provide care coordination, advocate for quality child-centered care and collaborate to design policies, procedures and programming that allow individuals and communities to develop to their full potential. The Registered Nurse Care Coordinator must be a self-starter; have strong organizational skills, and a high level of independent judgment required in decision making and duty performance.  The ideal candidate will be comfortable collaborating with individuals, families, and Community Based Organizations in order to implement and encourage participation in health and wellness programming ranging from health and nutrition workshops to wellness activities and parent workshops. He/she meets with supervisors and/or directors regularly; and performs related duties as assigned.

 

We are looking for an individual who truly cares about making our ECE programs a better place for our children, families, staff and the community at large. Felton currently serves approximately 400 children across five ECE sites in various neighborhoods in San Francisco.

 

Essential Functions:

  • Leading and maintaining Felton’s “Healthy Families, Healthy Lives” health and wellness program for children, families, and staff via the Bryant Street Weekly Farmer’s market offerings.
  • In collaboration with Family Support team, help children, families and staff attain an optimum level of physical, social, and emotional health via health, nutrition, physical activity, and emotional wellness
  • Developing plans for children’s medical needs based on assessment, interventions, and identification of needs.
  • Serving as a health and wellness consultant with ECE teachers, Child and Adult Care Food Program (CACFP) personnel, counselors, case managers and leadership teams.
  • Providing health-related education to children and staff in both individual and group settings
  • Monitoring and verifying immunization records, managing communicable diseases, and assessing the school environment as to prevent injury and ensure health, safety & wellness
  • Overseeing infection control measures
  • Actively participating in school safety plans that address school violence, bullying, and emergencies that may occur across all Felton’s ECE programs.
  • Overseeing medication administration, health care procedures, and the development of healthcare plans
  • Overseeing vision, hearing, dental, lead, body mass index screening procedures/ activities
  • Making decisions related to the delegation of healthcare tasks as directed by state laws and professional practice guidance
  • Providing health advice to school educational, early intervention and inclusion teams
  • Assisting families with locating outside health resources and obtaining health insurance

Job Description

 

Program Specific Responsibilities

  • Managing daily medical needs of children under MOLERA services:
  • Collaborate with Early Intervention Supervisor to collect necessary documentation to determine the eligibility for MOLERA services as outlined in the application process.
  • Participation in review of the MOLERA application for services to ensure agency capability and ongoing monitoring to ensure optimal care is provided while mainstreaming children with special medical needs in ECE classrooms across Felton’s program.
  • As part of the MOLERA services, develops and implements nursing care plans for medically fragile children in collaboration with the child’s family and medical team.
  • Maintains a caseload of 5 children with significant medical needs for whom she provides daily nurse monitoring as well as implementation of all medical procedures that these children require. Act as a case manager for those 5 children, working closely with EII team to secure any additional services the child may need.
  • Provides in-service trainings upon a MOLERA child’s enrollment to Teachers and Program Supervisors.
  • Holds at least six (6) yearly meetings (one every other month) with each MOLERA enrolled family to discuss medical updates, progress of nursing goals, and other necessary topics.
  • Twice a year, responsible for completing MOLERA progress and annual Reports
  • Responsible for keeping track of all services provided to MOLERA children on caseload

Health, Safety & Wellness Guidelines

  • Review and implementation of FSA policies which govern medical, health and safety procedures.
  • Responsible for creating, updating and advising Teachers, Program Supervisors, and Directors of health policy and procedures when needed. Act as a point of contact when medical situations or illness arise.
  • Provide guidance and consultation to Program Supervisors, staff across Felton’s ECE programs as needed for illness or medical related situations.
  • Responsible for leading all updates of Felton Medical Manual for treatments that classroom staff is allowed to provide after appropriate training (approved by Community Care Licensing and by California
  • Department of Education).
  • In collaboration with Administration Director create Emergency Preparedness policies and procedures and organize all safety drills at all Felton ECE sites. Safety drills includes: Fire drills, earthquake drills, active shooter drills etc...
  • Responsible for ordering all supplies, assembling First Aid kits for classrooms. At least every six months, conduct Health & Safety audits to ensure all First Aid supplies are onsite across all Felton ECE classrooms and sites. (First Aid Kits, Large First Aid Bins).

Health, Safety & Wellness Programming

  • Lead the prevention and control of communicable diseases across all ECE sites.
  • Coordinate a comprehensive program of caring for children who are injured or who become ill at school. Notify parents families and building administrators regarding children who are severely ill or injured while at school.
  • Lead prevention and intervention services including, but not limited to a. Vision screenings. b. Hearing screenings. c. Dental screenings d. Lead screenings e. Weighing and measuring. f. Follow-up of problems noted. f. Consultation with teachers and parents regarding health problems.
  • Responsible for the supervision and daily health care needs of the children across Felton's ECE programs. This would include daily rounds/check in with sites, updating classroom staff of medical situations to monitor, oversight of medicine dosages, making referrals for additional care, COVID-19 and general exposure notices, documentation of health status, and performance of specific medical procedures for children with special needs and other children.
  • COVID-19 Specific Responsibilities while the pandemic continues: Respond to, report, document, and follow up with COVID-19 cases in children. Identify children with COVID-19 symptoms and ensure protocol is followed for child’s isolation and return to school. Collaborate with SFDPH on best practices, and COVID regulations.
  • Documentation of treatments, changes in treatment plans, accident reports that require follow-up, communicable disease reports, child screening reports, and generation of funder /department of public health reports.
  • Ensure exposure notices are posted/ distributed and Facility Supervisor /Director is informed if outbreaks require special cleaning/disinfecting by emailing cyffacilities@felton.org.
  • Lead ECE sites health and wellness programming including: hand washing, dental hygiene, asthma
  • Lead and maintain Felton’s “Healthy Families, Healthy Lives” health and wellness program for children, families, and staff via the Bryant Street site Weekly Farmer’s market offerings including access to food, cooking demonstrations and educational brochures and resources.
  • Leads wellness workshops for the purpose of conveying and/or gathering information required to help sustain and improve wellness activities.
  • Responsible for reviewing, storing and overseeing CACFP special meal accommodations, menu offerings to ensure compliance with all CACFP requirements.

Data Collection and Record Keeping

  • Maintains organized and complete (medical) children’s files for each enrolled child, recording all health and medical data pertinent to the child’s health and wellness– Title 22 require that these be in one place. Codes labels of files so that California Department of Education files can be easily identified for review. Ensure all medical information is kept in a locked KIPPA compliant cabinet.
  • Coordinate a program of communicable disease control based on California’s immunization laws, to include: a. Maintaining immunization records for each child. b. Communicating with parents regarding immunization requirements.
  • Maintains current Immunization records for each enrolled child, including all funding sources. Completes annual (October) Immunization Report and submits to Department of Public Health and
  • Admissions, Data and Evaluation Director. Stores completed report on Box.
  • Responsible for monitoring non-compliant parents with regards to enforcement of delinquent immunizations/health screening will notify the Admissions Coordinator for issuance of Notice of Action to Terminate services.
  • Maintain record of, and carry out, standing orders for treatment of non-life-threatening illnesses.
  • Responsible for monitoring the “Classroom Medication Logs” for accuracy, and completion. Address any training needs as necessary. Ensure teachers maintain accurate records of parent/guardian signatures for medication distribution.

Education, Training and Capacity Building

  • Responsible for creating and distributing a monthly “Health & Wellness Newsletter” to support health, nutrition, physical activity, and emotional wellness
  • Coordinates the provision of Nutrition Classes, Healthy Cooking Demos and Immunization Clinics across Felton’s ECE programs.
  • Identification of training needs for staff and provision of such training in a structured, documented format as part of professional growth requirements.
  • Documentation and submission of trainings provided to staff, and public health issues which require notification to Administration or city health departments.
  • In coordination with Administration Director is responsible for the development of, and implementation of a minimum of 15 hours in service trainings each calendar year, to be logged in for verification of professional growth hours for classroom credentialed staff. (i.e., health and safety training, including but not limited to CPR, first aid, universal precautions, hand washing and diapering procedures, etc.… (using Felton’s Medical and New Employee Orientation guidelines, policies, and licensing/ state regulations).
  • Leads the Annual Resource Fair in collaboration with Family Support team, specifically targeting health, safety & wellness community resources for families. Support, assist, and coordinate health/wellness events at all Felton ECE centers.
  • Organize and create division -wide educational staff training for all Felton ECE Sites; collaborating with community organizations when needed.
  • Collaborate with Family Support Director to provide educational workshops to pregnant and parenting teen population.
  • Stay up to date with COVID-19, Health and Safety recommendations and guidance, and educate staff and families through virtual trainings, Zoom meetings, newsletters, etc.
  • Provides medical resources and referral information to parents
  • Assists in the opening of new ECE sites, ensuring all necessary health and safety procedures are in place. Additionally providing facility wide training at the new sites for proper medical policies and procedures.

Teaming and Collaboration

  • Act as the primary liaison between Felton and the Department of Public Health Nurses to maintain safety guidelines procedures across all Felton’s ECE sites. Ensure consistency and timely coordination of all health and safety policies and procedures.
  • Serve as liaisons between school personnel, family, and community healthcare providers to ensure a healthy school environment.
  • Assists with the interviewing and in-service training of EITA’s who will be working with MOLERA children, or children with medical needs.
  • Attend EII Team Meetings and Clinical Supervision as needed to collaborate and advise on mutual clients
  • Attend the CYFTAY Leadership meetings as needed to collaborate and share information pertaining to health and wellness programming.
  • Collaborates with various therapists (PT, OT, SLP, School Counselors, Clinicians) on the treatment of children.
  • Oversee the growth and development of Felton’s ECE medical team (including but not limited to interns, nursing students, Registered Nurses) as the organization grows in depth and breadth of health and wellness programming.
  • Performs other duties as assigned by supervisor or assigned 

Minimum Qualifications

  • Graduation from an accredited School of Nursing
  • Current California State Nursing License
  • BLS/ CPR First Aid Certification
  • Experience working with children birth to five and their families
  • Three years pediatric nursing experience- preferred*
  • Knowledge of child development and assessment
  • Demonstrated experience in diverse, multicultural and multidisciplinary professional setting
  • Communication and training skills for staff development
  • Must pass a Department of Justice fingerprint clearance and a Child Abuse Index, Health Screening and TB Test as required by licensing.
  • Commitment to social justice, empowerment of families and support for diversity.
  • Demonstrate quality organization, written and verbal communication skills.
  • Attention to detail and multitasking abilities
  • Must have demonstrated leadership skills and abilities.
  • Ability to work independently under broad supervision
