Job Summary:
This position will work as a member of the reproductive health team, which works to educate and direct patients to services regarding health, wellness, prevention and counseling in regards to reproductive health. This position performs a wide variety of clerical and support duties in support of patient access across multiple sites for the NYS Family Planning Program. An integral part of the reproductive health team acting in cross-functional fast paced environment in order to meet performance and quality assurance benchmarks.
Duties/Responsibilities:
- Maintain patient files within electronic medical system
- Reviewing provider notes for reproductive health visits
- Documenting Client visits reports
- Ensuring accuracy of client visit reports
- Enter/update electronic medical record with information from intake paperwork gathered by front desk staff
- Claim data set up for NYS family planning program
- Communicate with staff, providers and/or supervisors in order to ensure program adherence to expected documentation standards
- Provide documentation of noted deficiencies in documentation
- Ensure proper documentation of all interactions and outcomes
- Assist with Program Functions as needed and designated by Reproductive Health Program Manager
- Outreach to patients who are due for preventative health and wellness
- Assists in scheduling appointments for preventative care
- Takes part in Quality Improvement Projects and makes recommendations for improvements based on observations.

Education and Experience:
- High School Diploma or Equivalent
- 1 year clerical work preferred
Physical Requirements:
- Must be able to hear and communicate with clients and staff via phone, web media, and in person.
- Must be able to reach above shoulder level to work, bend, squat, sit, stand, stoop, and kneel.
- Must have vision which is adequate to read memo's computer screen, registration forms, and other documents.
- Must possess manual dexterity, to perform writing and keyboarding tasks.

Salary : $40,700 - $51,500
