First Resources is growing and we are searching for a dedicated Full-time Area Coordinator to oversee daily operations in Bloomfield, Centerville, and Knoxville.
The ideal candidate would be Responsible for the coordination, development, and oversight of all programs offered to ensure all aspects of HCBS services are provided in accordance with Person Center Service Plan (PCSP), Also, responsible to ensure all codes of the state, contractual provisions with funding bodies, and the company's policies and procedures are followed. Expected to possess a high level of accountability, ability to stay in close and timely communication with others, and acceptance of consequence of error. This position entails a high degree of accountability and consequence of error. Ability to stay in close and timely communication with others is crucial. A coordinator is responsible for the health and safety of individuals receiving services, money, agency assets, equipment, and personnel.
Essential Job Functions:
- Participate in the creation of a monthly activity calendar, with supervisor and client input. (ADS Supervisor)
- Schedule and facilitate regular supervisor/staff meetings to convey pertinent information in a timely manner.
- May be responsible for the coordination and development of the Person Center Service Plan (PCSP), for individuals which includes, but is not limited to:
- - Development of goals and objectives
- - Time frame for completion of goals and objectives
- - Scheduling and chairing Interdisciplinary Team meetings
- May be responsible for initial and ongoing contact with individuals including, but not limited, to:
- - Coordination of tours for prospective individuals
- - Orientation
- - Completion of all relevant paperwork, specific to the master file
- - Notify appropriate departments of individual information through verbal and electronic communications (electronic health record).
- Serve as liaison with families, other service agencies, residential providers, and other community resources. Monitor and advocate for individual rights and refer individuals to appropriate services in the community, as requested.
- Responsible for the coordination and development of the Person Center Service Plan (PCSP), for the individuals which includes, but is not limited to:
- - Development of goals and objectives
- - Time frame for completion of goals and objectives
- - Scheduling and chairing Interdisciplinary Team meetings
- In conjunction with Human Resources may be responsible for directly and indirectly provide supervision for the area staff by completing, overseeing, and/or assisting staff/Supervisors with the following:
- - Staff evaluations, performance measures, and disciplinary actions
- - Assist with developing, organizing, and facilitating training for area staff
- - Assist with the hiring process and orientation, relevant to staff's position
- Oversee funding authorizations while ensuring communication with funding bodies, and the Accounting Department, about any discrepancies while working to remedy issues.
- Responsible for developing, coordinating, and maintaining master files for individuals, as well as completion and distribution of Person Center Service Plan (PCSP), discharge documents, and follow-up reports, in appropriate time frames.
- Assist with the development of staff work schedules and duty assignments, ensuring adequate ratios and supports are maintained, related to client need.
- Responsible for creating a positive culture for the respective location(s).
- Works closely with Compliance Department to ensure resolution of any identified need and proactively works to mitigate risks or penalties.
- Provide oversight and quality checks of individual's personal accounts and payee accounts, at least monthly.
- Be an active participant of the Management Team of the Disabilities Department.
- Oversee the day-to-day operations of services in a specific area, ensuring adherence to all licensing/accrediting standards is satisfied (CARF, DIA, HCBS, etc.).
- Act as a liaison with parents, guardians, volunteers, and other contacts.
- Actively participates in/initiates communication with all internal/external parties so services for individuals are not interrupted due to lack of proper coordination.
- Supervise and evaluate staff; providing support and facilitating training as needed. Collaborate with supervisor for all disciplinary actions/performance improvement needs.
- Utilize the coaching tool with staff on a consistent basis to promote staff development.
- Follow direction from HCBS- Director on how to handle given case load based on location.
- Ensure that only non-aversive behavior management techniques are used as outlined in the behavior plans and report all incidents of suspected Individual's rights violations.
- Participate in the organization's safety program, which includes, but is not limited to, emergency evacuation and disaster procedures. Must be capable of executing emergencies and evacuation procedures. May provide transportation in the event of an emergency.
- Work closely with HR and immediate supervisor to identify staff vacancies, post job openings, and hire qualified staff members.
- Participate in the "on-call" rotation.
- Responsible for filling vacant shifts in a financially responsible way; working shifts as needed to minimize overtime.
Requirements:
- Bachelor's degree in a related field with at least two (2) years' experience working with people with disabilities or associate's degree in a related field with at least four (4) years' experience working with people with disabilities.
- Prior supervisory experience required.
- Must possess basic knowledge of HCBS and Habilitation programs.
- Prior supervisory experience preferred.
- Must have a valid driver's license, a good driving record and show proof of auto insurance.

Salary : $35,600 - $45,100
