Position Summary:
As a Case Manager, it will be your responsibility to build and maintain patient relationships throughout the patients care at FLEXX. You will be responsible for acquiring long term, quality patients for the clinic through the implementation of proven lead conversion systems. You will participate in educating the patients while working with the clinicians as they report the findings of an orthopedic exam and diagnostic imaging. It will be your responsibility to deliver a financial breakdown of the overall Care Plan along with helping patients to coordinate their schedules to arrive for the recommended care. As a Case
Manager, you will focus on understanding patient needs and wants, helping them overcome obstacles and empathizing with their struggles and challenges while influencing them to do what is in their best interest to improve their quality of life. To be a remarkable Case Manager, you must be able to develop trust and the confidence necessary to help patients navigate their Care Plan.
You will need to express a breadth of knowledge and experience in these areas, as well as the ability to work and collaborate effectively within a highly team- centric environment.

Performance Objectives:
● Develop emotional connection with patients to understand their expected goals and desired outcomes
● Perform an initial consultation on new patients to determine medical history, motivating factor that prompted patient to look for help with their problem and functional goals of the patient
● Attend Team Meeting to report on motivating factor that prompted patient to look for help with their problem and functional goals of the patient
● Create and deliver financial estimates and long-term scheduling to patients based on Provider’s orders
● Provide and ensure completion of all patient paperwork
● Effectively and continually educate patients about the need for, and
progress through, the care plan
● Ensure that all of the patient’s needs are met, nothing gets overlooked, and care is consistent with the patient’s expressed desires
● Execute patient follow up calls
● Encourage, resolve concerns, and gather testimonials during care plan

Education & Experience:
● High school diploma or equivalent
● 2 years of sales or management experience
● Experience working in a private medical practice preferred

Salary Range:
$15.00 - $20.00 base salary
