Job Title: RN (Registered Nurse) Care Manager
Reports to: Associate Director
Florida • CHF (Congestive Heart Failure) Clinic Care Management Program
**THIS IS A HYBRID ROLE THAT WILL WORK BOTH REMOTELY AND REQUIRE ON-SITE CLINIC VISITS, THEREFORE CANDIDATES MUST RESIDE IN THE IMMEDIATE TAMPA BAY REGION**
As an RN Care Manager, you will:
- Work in collaboration with the multidisciplinary health care team (which includes licensed clinical social workers, nutritionists, and pharmacists), cardiologists/nurse practitioners, and patients to serve as a patient advocate to support, guide, and coordinate care for patients, families and caregivers as they navigate their health and wellness journeys.
- Along with other members of the team, conduct comprehensive assessments that include the medical, behavioral, and social needs of the patient to identify risk drivers and barriers to attaining improved health
- Create and implement a care plan that reflects the goals and priorities of the patients, and addresses the identified needs
- Partner with patients to anticipate and address challenges in implementing care plans, including navigating complex systems and nonlinear pathways
- Collaborate with cardiologist and other members of the health care team
- Provide face-to-face interaction with patients and their care team when appropriate.
- Is fully accountable for the completion of work within established timeframes
- Employ established techniques such as motivational interviewing, harm reduction and trauma informed care to meet people where they are, create safety, and explore health behavior change
- Participate in weekly care team huddles
- Support seamless transitions for patients and their family/caregiver(s)
- Serve as primary point of contact for patients
- Work in a hybrid model with some travel required for in person patient meetings at cardiologists' offices. The hybrid work environment consists of home office, cardiologists' offices, and clinic time.
- Adhere to policies and procedures, regulatory standards, and documents according to standards
- Complete other essential duties as assigned

Requirements
- Current unrestricted Florida RN license
- 5 years of clinical experience (Hospital and/or office/clinic or care management)
- Current valid drivers' license, and access to insured vehicle
- BLS Certified

- Certified Case Manager (CCM)
- Bilingual (English and Spanish)
- Experience working with cardiac patients
Knowledge/Skills:
- Expert computer skills with experience and comfort using technology for virtual communication, scheduling, and documentation (Including Microsoft applications)

Work Environment:
The employee works in a HIPAA compliant work-from-home environment. Travel to clinic locations will be required. May be required to work extended hours and perform under time pressure to accomplish strict deadlines. The employee must be flexible in working in different scenarios and adapt to schedules according to operational needs.
