LTSS CASE MANAGER - INTAKE

The Long Term Services and Supports (LTSS) Intake Case Manager is responsible for screening a member an determining if an assessment is appropriate, completing the assessment, and pre-planning services. The LTSS Intake Case Manager is also responsible for referring activities for people with long term care needs; this could include people with intellectual disabilities, who are elderly, blind, physically disabled or living with brain or spinal cord injuries or people with significant mental health needs. LTSS Intake Case Manager is also responsible for assessments for hospital admissions/discharges, PACE program, and PASRR’s. Spanish bilingual preferred with a $1.00 increase in pay rate.

Requirements:

· Bachelor’s degree required; or five years of relevant experience in the field of Long Term Services & Supports, which includes Developmental Disabilities.

· Driving is a requirement for this position.

· Prior case management and/or advocacy experience strongly preferred.

· Knowledge of forms and process for Long Term Medicaid services preferred.

-Remote/Mobile position- must be able to attend trainings and meetings in Fort Collins, CO and throughout Larimer County.
