Part-time Social Worker (BSW)

Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a weekend Bachelor's level Social Worker.

Key Job Duties and Responsibilities:

• Initiates discharge planning and arranges aftercare appointments/referrals.
• Completes all needed discharge paperwork with patients and supports at time of discharge.
• Completes psychosocial assessments on new patients within the required timelines.
• Engages in collateral contacts with community organizations, support persons, and other providers and appropriately documents the contacts.
• Attends appropriate team meetings as needed.

Hours: Day shift, every other weekend.

• Bachelor’s Degree in social work from an accredited university
• LBSW, or ability to abtain license within 90 days of hire
• Previous psychiatric experience preferred
