What are the responsibilities and job description for the Care Navigator, Forsyth Tech Cares Program position at Forsyth Technical Community College?
The Forsyth Tech Cares Office provides holistic, wrap-around services and resources to help students address barriers that prevent them from attending and completing school. These services are provided in collaboration with both internal and external partners. The mission is to reduce obstacles to education by serving as a resource to address the information gap for students who qualify for community and public assistance programs. These include benefits and services such as tax credits, health insurance, food stamps, child-care, financial counseling, legal assistance and other essential services. The Care Navigator is responsible for being the administrative anchor of the Forsyth Tech Cares program and serves in a leadership capacity within the Student Support Services sub-division. The Care Navigator will act as advocate and help students foster their capacities and coping mechanisms in order to best assist them in achieving their educational goals. The Care Navigator will also plan, coordinate, manage and implement support services offered by the Forsyth Tech Cares Office.

Minimum Qualifications

- Bachelor’s Degree or higher. NOTE: Degrees and hours must be from a college accredited by a United States Department of Education accepted accrediting agency such as SACSCOC.
- One year of experience in social work.
- Strong work ethic, character and personal integrity, and ability to work with the utmost professionalism, discretion, and confidentiality. Conducts student intake and assessment for Single Stop clients and facilitates referrals to partnering service providers.
- Experience working with individuals facing critical needs
- Crisis management experience.
- Knowledge of Single Stop Software or Outcome Tracker.
- Develop and implement programming, services, and learning opportunities for students to promote the personal, professional, and financial development of a diverse student population.
- Coordinate integration of partnering service providers, and cultivate new partnerships (e.g., city agencies, not-for-profit organizations, external and internal organizations, etc.) into holistic, wrap-around services by monitoring the scheduling of appointments and facilitating referrals to support student development and growth.
- Facilitate workshops, support student programs, build resource and partnership library, develop all marketing materials, maintain the website, and manage all other administrative areas of the Forsyth Tech Cares Office.
- Manage all food, financial, and housing insecurity programming for the College.
- Conducts student intake and assessment for students and facilitates referrals to partnering service providers.
- Determines nature of student’s situation by interviewing, assessing medical, psychological, emotional and social information and making on-site visits.
- Establishes course of action by exploring options and setting goals with students.
- Obtains assistance for students by referring him/her to community resources, arranging for appointments and establishing rapport with other service providers.
- Fosters student’s action or adjustment by interpreting attitudes and patterns of behavior, explaining and suggesting new options.
- Maintains records of case by documenting situations and actions.
- Manages and ensures that follow-up with both students and partnering service providers occurs regarding referrals and outcomes.
- Maintains and ensures the proper documentation of all student data including student contact, referrals, and outcomes are documented in a confidential, accurate, and timely manner.
- Maintains operations by following policies and procedures, participating in quality reviews and reporting needed changes.
- Complies with federal, state and local legal requirements by studying existing and new legislation, enforcing adherence to requirements and advising department on needed actions.
- Maintains student confidence and protects operations by keeping information confidential.
- Contributes to team effort by accomplishing related results as needed.
