PRIMARY PURPOSE
: The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring patients' health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members. Is to be clinically competent, deliver care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patient, significant others, and other health team members while maintaining standards of professional nursing. The RN collaborates with the Practice Manager for the assigned Department and directly reports to the Director of Nursing. The Registered Nurse is responsible for clinical supervision in our health center while reporting the Director of Nursing.
ESSENTIAL JOB FUNCTIONS:
  • Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within timeframe specified by client facility’s policies, procedures or protocols.
  • Conducts ongoing assessments as determined by patient’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
  • Develops plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
  • Performs appropriate treatments as ordered by physician in an accurate and timely manner.
  • Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family. • Delivers care with appropriate age and cultural competence to specific patient populations according to individualized needs.
  • Provides individualized patient/family education customized to the patient and his/her family.
  • Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
  • Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
  • Maintains confidentiality in matters related to patient, family and client facility staff
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity and rights.
  • Reports patient condition to appropriate personnel during each shift.
  • Maintains current competency in nursing specialty by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
  • Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient's care and leading Medical Assistants and other health care team staff in meeting patient health outcomes.
  • Assess the patient; collect and document the patient's current health status and medical history. Make accurate and ongoing assessment of patient's status and respond appropriately.
  • Plan/Implement; set priorities for nursing action in a logical sequence according to patient's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. Plan includes physiological, psychosocial, and environmental factors, patient/significant other education.
  • Communicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, Collaborate with other health team members to ensure the patient's well being as evidenced by staff/client feedback.
  • Teach patient/significant other appropriate health information in a timely manner and share written informational material.
  • Perform direct patient care, utilizing established procedures, policies, and standards.
  • Administer medications according to policy and procedure.
  • Observe and document patient's response to pertinent medications.
  • Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure (done within a timely manner).
  • Serve as a critical member of health care team by demonstrating the ability to handle emergency situations in a prompt, precise, and professional manner.
  • Provide personal patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.
  • Maintain a clean, neat, and safe environment for patient/staff and assist Practice Manager and Medical Director with addressing health center issue related to patient flow, medication refills, etc.
  • Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner.
  • Demonstrate correct and safe technique in the use of equipment according to specific product information.
  • Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.
  • Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.
  • Remain conscientious in regard to personal hygiene and comply with specific division/unit regulations.
  • 100% attendance to mandatory skill updates yearly required.
  • Communicate all concerns to the Director of Nursing and or Chief Medical Officer.

  • Must also be able to respect the culture, values and opinions of others.

  • Incumbents may be required to perform other job-related tasks other than those specifically presented in this description.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Knowledge of N procedures.

Ability to orally communicate effectively with others, with or without the use of an interpreter.

Ability to communicate effectively in writing using the English language, with or without the use of auxiliary aids or services.

Ability to review, understand, and apply concepts presented in training programs, conferences, and/or professional literature.

Clear understanding that sister agency CAP program, provides information on educating individuals on safer sex practices which would include but not be limited to, exposure to explicit language, explicit printed material, and descriptions of explicit sexual activities as part of the agency’s mission in the prevention and transmission of HIV disease.

PHYSICAL REQUIREMENTS:

Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.

Need to travel to off-site locations and attend meetings, workshops, seminars plus travel to other Foundcare/CAP departments and Foundcare/CAP conference rooms.

Accomplish job duties using various types of equipment/supplies, e.g.
pens, pencils, calculators, computer keyboard, telephone, etc.

MINIMUM QUALIFICATIONS:
Graduate of an accredited school of Nursing.
Minimum one year of medical experience in a community health center environment or equivalent combination of training and experience, including supervision of other staff.
Valid registration as a Registered Nurse in the State of Florida.
Current life support training.
Previous experience in patient care setting preferred and the ability/skills in coordinating health center activities.
Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment.
Maintain client and family confidentially.
