Job Title: Self-Sufficiency Case Manager (Full-Time Non-Exempt)
Type of Positions: Full Time; Sick & Vacation; Health Insurance Reimbursement; WY Retirement
General Description: This position reports directly to the CSBG Executive Director. Responsible for activities designed to provide strength-based case management for low-income individuals and families for the purpose of removing barriers to achieving self-sufficiency.

• Provide telephone and/or face-to-face screening.
• Explain agency services and determine program eligibility.
• Provide problem-solving assistance for clients in crisis.
• Assess client needs and levels of functioning. Obtain written permission from client to make referrals, coordinate delivery of services, and/or facilitate linkages to other appropriate agencies and services.
• Support clients through monitoring and evaluation of their case plan.
• Maintain accurate, complete, up-to-date documentation of client service activities.
• Research community resources to assure a wide knowledge of available service options and make information available to clients.
• Work to establish partnerships and engage other agencies in referring eligible clients.

Education/Experience

• High School Diploma or Equivalent.
• Motivated to grow professionally.
• Experience in Social Services/Case Management.
• Preference given to those who hold an Associate/Bachelor degree in Social Service field of study.

• Knowledge of social and economic issues surrounding poverty. Knowledge of community resources and agencies providing services needed by the homeless population.
• Ability to be culturally sensitive to diverse client populations.
• Ability to provide leadership, facilitate groups, and work cooperatively with staff and volunteers.
• Ability to establish boundaries ensuring professional and ethical interactions; adhering to established policies and procedures.
• Must possess computer and word processing skills and be willing to learn other specialized software.
• Ability to define problems, collect data, establish facts, and draw valid conclusions.
• Requires Local Travel and Out-of-Town Travel
• Must be Skilled in Oral and Written Communication
• Must be Dependable and Have Strong Interpersonal Skills
• Must be willing to submit to and pass a background check.
• Perform other duties as requested by the Self-Sufficiency Program Director.

Pay: $41,000.00 - $45,000.00 per year
