Reentry Program Case Manager - Position Summary
The Reentry Program Case Manager offers structured support and guidance to youth while on probation or incarcerated at SMC Juvenile Hall or Camp Kemp, when needed. Case managers will be conducting 1:1s to recruit for the program. Upon release, case managers work with youth to set goals around education, building life skills and constructive uses of time, in order to prevent recidivism.
Brief Description of the Reentry Program: The Reentry Program empowers youth to decrease juvenile justice system involvement and successfully transition back into their communities.
Program Components:
When needed, facilitate interactive CBT-based legal education curriculum, including weekly one-and-a-half-hour sessions and key experiential components (e.g. role plays, juvenile system stakeholder visit, field trip, recognition ceremony) that provide:
Knowledge: Information on legal and social consequences of juvenile crimes and legal system and experience with youth that have been impacted by gangs
Safety: Staff create safety for the group so youth can try new behaviors and identities
Community: Youth start change cycle within themselves which leads to broader change for their families and communities
Empathy: Positive experience of juvenile justice system and its members (Judges, DAs, PDs, victims)
Contemplation: Information on and examples of positive future paths to self-sufficiency
While youth are In-Custody and preparing for Reentry Services/Community Integration:
Individualized youth support inside the San Mateo County facilities; hours will vary based on individual youth needs
Cross-program collaboration and communication with all SMC facilities staff to provide optimal support for each youth
Conduct youth 1:1’s and perform assessments to determine youth program eligibility
Attend Multi-Disciplinary Team (MDT) for in-custody youth that have upcoming release dates
While youth are Out-of-Custody/Community Integration: 9 months of individual intervention, addressing criminogenic factors and allowing youth to experiment with new identities and build life skills in a safe and supportive environment
Intensive individualized youth advocacy: Based on Critical Time Intervention (CTI) model with phases for caseload dosage (2-4 hours minimum per youth per month based on phase in the program)
Multi-Disciplinary Team (MDT) Representation: Attend regular MDT meetings, as appropriate.
Attend court dates for out-of-custody youth and report on progress made in Reentry Program
Primary Responsibilities
Provide ongoing case management for incarcerated youth
Meet with youth regularly to build rapport and conduct assessments
Support and coach youth as they prepare for Reentry services and community integration
Enter data to track youth progress throughout the program
Plan Activities for Clients
Identify and collaborate with representatives from community-based organizations to plan youth activities
Build and maintain stakeholder relationships (i.e. representatives from community based organizations, corporations and donors)
Prepare agenda for activities and projects
Provide clients with transportation to and from activities, probation-ordered programs and events as needed.
Provide ongoing case/crisis management for 10-15 Reentry Program Youth
Meet with each youth on caseload for 2-4 hours per month depending on the phase of the program (this includes both group and individual contacts)
Provide crisis management and support to clients as needed. Client issues often include the following: depression, education, employment, family, gangs, homelessness, incarceration, substance abuse, self-esteem, suicide, and violence
Conduct intakes with each youth on caseload (which will include a Service Learning Plan and a variety of assessments)
Assist clients and families with locating and receiving outside services to address identified needs
Provide general Programmatic and administrative support
Engage In Community Meetings/Events
Identify ways to participate and engage in community events and meetings to understand the needs of the community in which the clients live
Attend and actively participate in regular community meetings, MDT’s, as needed
Compensation
This position is a full-time non-exempt role at a rate of $24.65/hour with differential pay options based on an advanced degree and speaking another language when applicable to the role. Our Total Rewards Package also includes the following:

COVID-19 Vaccination
At FLY, we are committed to the safety of our employees, clients, and all others who enter our workplace. While COVID-19 threatens the health and well-being of all individuals, vaccinations can significantly reduce that threat and help protect our workplace and community.
