JOB DESCRIPTION:
Our growing integrated treatment clinic is seeking and EXPERIENCED and CERTIFIED KY Targeted Case Manager to provide services to individuals with Co-Occurring Mental Health or Substance Use Disorders and Chronic or Complex Physical Health Issues (SMI, SED, OUD). The TCM will work under the direct supervision of the program Director and Operations Manager and will be able to:
COMPETENCIES:
Provide clients with a single point of contact for multiple health and social services systems
Advocate for the client
Able to be flexible, community-based, and client-oriented
Assist the client with needs generally thought to be outside the realm of substance abuse treatment
Understand various models and theories of addiction and other problems related to substance abuse
Able to recognize the importance of family, social networks, community systems, and self-help groups in the treatment and recovery process
Understand the variety of insurance and health maintenance options available and the importance of helping clients access those benefits
Understand diverse cultures and incorporating the relevant needs of culturally diverse groups, as well as people with disabilities, into clinical practice
Understand the value of an interdisciplinary approach to addiction treatment
Able to communicate effectively and participate in a multi-disciplinary team environment
Able to complete intake and needs assessments, service plans, reassessments, follow ups and mandated documentation in a timely manner
QUALIFICATIONS:
Must have a Bachelor’s of Arts or Science Degree in a Human Services related field (e.g. Psychology, Social Work, Counseling, Nursing, etc..)
2-3 years of experience (with a strong emphasis on addiction)
A strong understanding of Case Management relating to Co-Occurring Disorders
Attention to detail and organization a must.
Computer literate with experience in a Windows environment.
Ability to work independently with minimal supervision.
Job Type: Full-time
Pay: $38,000.00 - $45,000.00 per year
