Hospice Nurse

Overview: We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Home Health RN, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes. Responsibilities: - Conduct patient assessments and develop individualized care plans - Administer medications and treatments as prescribed by physicians - Monitor and evaluate patient progress, making necessary adjustments to care plans - Provide education and support to patients and their families - Collaborate with interdisciplinary healthcare team members to ensure comprehensive care - Perform various nursing procedures such as wound care, ostomy care, and tube feeding - Collect and process specimens for laboratory testing - Assist with behavioral health management and counseling Qualifications: - Valid Registered Nurse (RN) license in the state of California - Minimum of 5 years of nursing experience and 2 years hospice experience, preferably in home health or a related field - Strong knowledge of anatomy, physiology, and medical terminology - Experience with patient assessment and development of care plans - Proficiency in performing nursing procedures such as ostomy care and tube feeding - Knowledge of behavioral health management techniques - Ability to work independently and make sound clinical judgments - Excellent communication and interpersonal skills Skills: - Specimen collection processing - Behavioral health management - Tube feeding procedures - Strong anatomy knowledge - Experience with Botox administration (preferred but not required) - Patient assessment skills (including ER experience) - Knowledge of durable medical equipment (DME) - Familiarity with hospital protocols Join our team of dedicated healthcare professionals and make a difference in the lives of our patients.
