Memory Care Partner (MCP)
JOB DESCRIPTION
The MCP takes care of the personal needs of the resident in the memory care neighborhood in accordance with state and federal regulations, under the supervision of a licensed nurse. The MCP provides assistance with the individual activities of daily living for residents with memory loss who are assigned to their care, including assistance with dining, activities, and overall social wellness.
QUALIFICATIONS
Minimum Education Required: Successful completion and certification as a Certified Nurse’s Aide Certified Home Health Aide in the state of New Jersey.
Education Preferred: High School Diploma or Equivalent
Minimum Experience Required: 2 years
Experience Preferred: 2 years Long Term Care experience and specialized care experience
Licensure/Certification/Registration: Current New Jersey Certified Nursing Assistant Certificate, Certified Home Health Aide, and Certified Dementia Practitioner Training
Specialized Knowledge/Skills: 1) Good interpersonal skills, 2) Ability to work as part of a healthcare team, 3) Ability to communicate observations accurately, 4) Ability to relate to the elderly and cognitively impaired. 5) Ability to establish a trusting relationship with memory impaired residents. 6) Ability to provide person-centered-care.
Specialized Memory Care Partner Qualifications
a. Provides a person-centered approach to caring for people with cognitive difficulties.
b. Prevents negative behavior caused by inappropriate environmental stimuli.
c. Personalizes care needs specific to resident preference.
d. Maintain a specialized daily routine for each Memory Care resident.
e. Address and advocate for memory care residents’ physical, social, and emotional needs.
f. Possesses the following characteristics as a memory care partner; patient, adaptable, attentive, intuitive, observant, emotionally intelligent, proactive, and empathetic.
g. Engages and is comfortable leading in Life Enrichment events, assists with encouragement to participate.
h. Employ de-escalation techniques when necessary.
i. Act as a role model for teammates by providing exemplary care.
