Job Summary:
Responsible for coordinating the care of patients and their families or significant others within the emergency room (ED) setting. Monitors admission status, medical appropriateness, and coordinates discharge planning. Other duties as assigned.
Responsibilities:
A minimum of 2 years of experience in an acute care setting is required. ED/Trauma experience and casework in medical social work is preferred. Master's Degree in Social Work. Current State of Wisconsin licensure as a Social Worker.
Other information:
Interpersonal skills that are necessary to obtain information from patients to provide counseling to patients/families, and interact effectively with patients, families, and hospital personnel. Analytical skills necessary to assess patient's needs to develop plans associated with the patient's discharge and to be able to develop solutions to problems concerning patient's mental and physical wellbeing.
