If you want to have a positive impact in the lives of others come join us! PACT is an intensive and highly integrated approach for community mental health service delivery. The multidisciplinary team shares a caseload of individuals who experience symptoms of the most serious forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness. The Senior Clinician is a member of the Program for Assertive Community Treatment Team (PACT), a multidisciplinary team that meets fidelity standards for the evidence-based practice of Assertive Community Treatment. The Senior Clinician works closely with, a prescriber, nurses, clinicians, co-occurring specialist, peer support specialist, housing specialist and vocational rehabilitation specialist to serve a shared caseload of individuals who experience symptoms of a thought disorder and whose level of need exceeds what traditional outpatient services offer. The team provides case management, individual and group therapy, initial and ongoing assessments; employment and housing assistance, family support and education, substance abuse services and other services and supports critical to an individual’s ability to live successfully in the community. The Senior Clinician assists with training of new employees, provides guidance to team members and assists the supervisor as needed. ESSENTIAL DUTIES & RESPONSIBILITIES includes but are not limited to the following: Meet minimum direct service percent time as defined by management. Develop individual treatment plans based on the consumers’ strength, needs, hopes, and desires. Plans are modified as needed through an ongoing assessment and goal setting process. Provide pro-active assistance to consumers to participate in and continue treatment, live independently, and recover from their disability. 80% of direct services must be provided within community settings such a person’s own home and neighborhood, local restaurants, parks, and nearby stores. Coordinate care management of consumer’s substance and mental health needs including monitoring of individual plans of care and achievement of plan of care goals. Work with PACT team members to collaboratively provide treatment, rehabilitation, and support services required by each consumer to live in the community. Assist consumers to participate in community employment. Work with consumers and their family members/significant others to become collaborative partners in the treatment process. Include consumers’ natural support systems in treatment, educating them and including them as part of the PACT services. Provide health education, access, and coordination of health care services. Help consumers become less socially isolated and more integrated into the community by encouraging participation in community activities and membership in organization of their choice. Assist in development and delivery of staff and/or consumer education in the form of group classes or trainings. Coordinate with external evaluator as appropriate. Perform FBH charting requirements, correspondence, and other paperwork in a complete and timely manner. Provide outreach assessments of referrals for diagnostic eligibility, complete initial enrollment paperwork, open client’s charts in CCP and engage in discharge planning from inpatient settings and transitioning onto the PACT Team. Provide support and education to team members on CCP. Provide support and guidance with crisis situations and case consultation. Act as Team Leader in Team Leader’s absence. Assist teammates in completing Critical Incident Reports. QUALIFIED CANDIDATES WILL HAVE: REQUIRED: Master’s degree in social work, psychology, counseling or related field from accredited college or university. Two years of experience working with psychiatrically impaired populations. Must apply for Agency Affiliation within 30 days of hire. Must have reliable transportation, insurance and an active drivers’ license and be willing to use personal vehicle in the course of work. PREFERRED: Previous supervisory experience. Licensure in the State of Washington.
