What are the responsibilities and job description for the Community Health Worker position at Gallatin County, MT?
Work alongside the Gallatin City-County Health Department (GCCHD) Public Health Home Visiting Program (PHHV) and collaborate with community partner organizations to connect Spanish-speaking families to local resources and conduct primary prevention for substance misuse. Provides families with short term support while awaiting parenting education services from home visitors. This position is grant funded from 6/1/2023 to 12/31/2023.
Essential Duties:
- Assesses family needs based on historical and current information.
- Establishes partnerships with families as a service provider and advocate. Builds trust in the relationship between family members and the incumbent.
- Provides resource coordination to other services in Gallatin County, such as healthcare, mental health, social services, and the public education system for Spanish-speaking families in Gallatin County who have children aged 0-5 and are currently on a waitlist to receive support services through the home visiting program.
- Makes referrals when there is an opportunity for improving a family's life, health, or safety. Such referrals are made after discussion with the family, assessment, and consideration of the benefit of providing care coordination.
- Conducts evidence informed substance misuse focused screenings, provide families with culturally appropriate substance misuse prevention education, and assist in accessing relevant community resources.
- Completes required documentation in an accurate and timely manner for the purpose of keeping current and progressive data related to needs, efforts, progress, outcome, and communication with family. Enters data into Patagonia, GCCHD’s electronic medical records system.
- Provides monthly updates and communication with mentor and program manager about project implementation, progress towards goals, barriers, and successes.
- Confidentiality is maintained in all written and oral communications, in accordance with the Uniform Health Information Act or the Health Information Portability and Accountability Act (HIPAA), whichever is more stringent.
- This position is a mandatory reporter for child abuse and neglect.
- Translates Look Closer Toolkit from English to Spanish as well as other Maternal Child Health or Substance use resources as needed to benefit clients
Other Duties:
- This position has no regular supervisory responsibility over other employees.
- Working relationships: establishes and maintains relationships with health care providers, home visiting team, project manager, co-workers, and community partners and organizations to provide comprehensive support for Spanish-speaking client families.
- Physical/environmental demand: Work is performed in a wide variety of settings with clients and patients. Conducting home visits requires travel in inclement weather. Contact with the public in health care situations requires the incumbent to exercise universal precautions to prevent or minimize exposure to illness and disease. This position requires the incumbent to make home visits, often to individuals and families who are under stress, mentally ill, or would be considered high-risk for other psychosocial reasons. At times the incumbent would be required to handle contentious situations or angry public in a professional manner. The work requires moderate physical effort or exertion when carrying equipment or educational materials.
QUALIFICATIONS:

- Basic knowledge of social support systems
- Advocate for clients’ needs
- Function without judgement, bias, or stereotypes
- Facilitate clients’ access to health and social services
- Communicate with linguistic and cultural proficiency (e.g., in writing, orally, and visually)
- Explain health care payment mechanisms and procedures (e.g., Medicaid, Medicare, private insurance)
- Incorporate ethical standards of practice into all interactions with individuals, organizations, and communities
EDUCATION & EXPERIENCE:
- Fluent in Spanish and proficient in English
- Basic computer skills
- Completion of Community Health Worker training within two months of starting
- The required knowledge, skills, and abilities are typically acquired through a combination of education and lived and/or professional experience in any of the following areas:navigating social services
peer support
health, behavioral healthcare, health education
working with marginalized populations, indigenous communities, and/or economically disadvantaged populations
Preferred Qualifications
- Valid driver’s license and reliable form of transportation
- High school graduation or GED
