Hiring pay range: $32.98 - $41.22

Full pay range: $32.98 - $49.47

Supervision

Directly supervises and oversees a team of case managers. Executes supervisory responsibilities with fairness and accountability. Strives to manage and resolve conflicts and works with team and staff to meet goals and expectations. Encourages training and development, actively coaching and empowering employees.

GENERAL PURPOSE OF JOB:

Supervise a team of case managers who provide initial and ongoing case management services to members accessing and/or receiving Medicaid Long Term Care benefits and services in Garfield, Summit, Eagle, and Pitkin Counties.

ESSENTIAL JOB FUNCTIONS:

Essential Functions

Supervision:

  • Supervises staff: schedules, assigns and directs work; Provides regular direction and scheduled 1:1 supervision time to each employee. Completes performance evaluations.
  • Conducts interviews; hires and orients new employees; oversees or conducts training; coaches; evaluates and appraises; addresses complaints and resolves problems; takes disciplinary action; and ensures quality of work.
  • Ensures compliance with County and Department policies and relevant Federal, state and local laws, rules, regulations, codes, statutes, and CMA contract.
  • Organizes and schedules workflow, including maintaining adequate work coverage.
  • Reviews documentation completed by staff; checks work for accuracy; and edits and approves documents and system input.
  • Reviews case record to approve a notice of action for members who have been functionally approved, denied, and/or have program or service changes.
  • Plans and reviews short-term and long-range goals and objectives and participates in annual goal setting.
  • Ensures that staff has received required initial and ongoing training. Acts as back-up for staff and other program supervisors.
  • Ensures that individuals and team meet goals and provide highest level of service.
  • Part of a supervisor team that addresses program related delivery of services.
  • Performs other duties of a similar or related level and assists with special projects as necessary or assigned.

Program Management:

  • Assists and informs Program Manager with contract compliance, monitoring, and oversight of program contracts.
  • Monitors case managers’ work for quality and accuracy in accordance with program policies and procedures.
  • Identifies funding opportunities, and assists Program Manager in grant writing, as assigned.
  • Receives and synthesizes reports and data and assists in preparing a monthly report for the BOCC and with completing monthly contract reports.
  • Assist with responses to HCPF reporting requests and requirements such as the QIS remediation, contract deliverables, and review of records.
  • Works closely with employees and program manager to develop appropriate responses and make critical decisions. Consults with the County Attorney’s office related to legal questions and legal ramifications of responses and decisions.
  • Understands program area budget. Authorizes expenditures of funds for services that meet a member’s needs and practice fiscal responsibility. Has expert knowledge of funding sources and accurately allocates from sources.
  • Approves employee data entry in the data management system-member assessments, certification record, service plans, care plans, log notes, and CCM uploads.
  • Assures a complete record with updated signed paperwork is available in Laserfiche and as required in CCM.
  • Maintains strong working knowledge of applicable laws and regulations related to program area, best practices; applies knowledge in development, implementation, and evaluation appropriate policies, procedures, and processes to ensure highest level of professional service to members.
  • Participates, as needed, in meetings and other activities with members to assist in assessing ongoing needs and monitoring progress toward goals.
  • Participates in Department, county, or community meetings; networks with outside providers and agencies to ensure optimal use of community resources.
  • Works directly with staff of other Department divisions to ensure members’ needs are met.
  • Provides “shadow” supervisory visits with case management staff. This includes a minimum of two visits per year for each staff member.
  • Provide follow-up with members to ensure case management services are meeting member needs and case management staff are performing as expected.
  • Administer state-funded Family Support Program and/or assist case management staff in the administration of the Family Support Program.
