What are the responsibilities and job description for the RN Program Lead - GHP Care Coordination position at Geisinger?
- Leads program implementation and evaluation of the plan and provides reports relevant to achievement of goals.
- Provides oversight to ensure area care delivery, quality management
- Develops, implements, and maintains the philosophy, objectives, policies, and standards for the care of patients.
- Provides interpretation of the regulatory standards and/or program protocols.
- Creates and manages long-term goals.
- Develops a budget and operating plan for the program.

Qualifications:
- Hours: 8AM to 4:30PM
- Travel: Less than 50%. Must be a PA resident willing to travel throughout the Geisinger footprint as needed
- Preferred experience: Case Management or Home Health
