Responsibilities

Job description

  • Supervise staff that calls your patients monthly and as needed.
  • Establish a relationship and build credibility with the patient and their families.
  • Effectively communicate with patient's providers and broader care team.
  • Ensure that patient is receiving the appropriate care, guided by provider's care plan and your experience.
  • Provide ongoing education to help the patient better manage their chronic illnesses.
  • Create and update comprehensive care plans and care summaries.
  • Document all communication and coordination of patient care.
  • Follow all privacy policies and maintain the confidentiality of protected health information.

Job Type: Full-time

Salary: $32.00 - $38.00 per hour
