Position Summary
An Educational Service Coordinator's p rimary responsibilities include, but are not limited to developing residential treatment plans, facilitating treatment team meetings, acting as a liaison with schools, and coordinating needed services.
DUTIES AND RESPONSIBILITIES
I dentify CPST activities for each youth on caseload, individualized to their mental health needs that will lead to improved success in the community, school, work, family, or support access to needed services.
This can be delivered individually or in a group setting, face-to-face, over the phone or via teleconference. Location of service should meet the needs of the client.
Focus activities on one of more of the required CPST areas from OAC rule 5122-29-17.
Ensure invitations are sent timely and maintain documentation of all records related to Service Plan Review (SPR) meetings and the CANS Assessment Tool.
Scan documents into the Freemed system and turned in for filing.
Facilitate and complete clinical documentation of SPR meetings for youth in residential treatment at Genacross Family and Youth Services.
Gather information for Service Plan reviews and monthly reports for clients, including but not limited to on and off campus activities, visitations, behavior objective log data, appointments, educational progress, and review plans and obtain signatures.
Participate in IEP / ETR meetings as needed. Support the maintenance of educational records, including but not limited to activities such as entering quarterly grades and scanning in IEP / ETR documents.
Performs duties as assigned by Director of Education and Youth Services or other administration.
Provide training as requested by Administration.
EDUCATION, TRAINING AND EXPERIENCE
Demonstrate the ability to organize, manage and execute responsibilities under limited time frames within a fast paced and changing environment.
Strong organizational skills and ability to complete tasks as directed by Director level personnel.
Past direct child and youth care experience in a residential program at Genacross Family and Youth Services preferred. Clinical license preferred.
Possess outstanding written and verbal communication skills. Be proficient in Microsoft Office software and Freemed. Able to communicate with all levels of the organization.
Must possess or secure and maintain certification in First Aid, CPR, and a valid driver license with a driving record that is acceptable to LHS F&YS insurance company.
Comply with Genacross Transportation policies and procedures and is able to work flexible hours.
A Bachelor's degree in education, social work, psychology or related field is required.
Excellent written and oral communication skills.
Great interpersonal skills.
A passion for caring for children.
Must be 21 years or older.
Must successfully complete all background screening requirements.
Valid Driver’s License.
Preferred First Aid / CPR / AED certified.
