What are the responsibilities and job description for the Nurse Practitioner position at GHC Hospice?
Job Summary: Works in collaboration with the Medical Director and Hospice Team to provide Hospice care services to patients and families in the inpatient setting or in a home setting within the scope of practice of a nurse practitioner. Perform procedures for which he/she is privileged to do and documents assessments, evaluations, consultation reports and progress notes.
Demonstrates Competency in the Following Areas:
- Performs face-to-face encounter with the patient as a part of the certification process; completes Attestation documentation.
- Collects and assesses patient care data, develops and implements a plan of care, manages and evaluates outcome of care.
- Performs physical and psychosocial assessment on all Hospice patients.
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain management plan when pain is not manageable on the current medications.
- Demonstrates the ability to prescribe medications (see individual state laws) and non- pharmacological therapies based on the patient's diagnoses, health history and age. Regulatory Requirements:
- Current Registered Nurse License in the State of Georgia.
- Master’s Degree from an accredited Nurse Practitioner's Program.
- National Certification as a Nurse Practitioner or certification eligible.
- Current BCLS Certification.
- A minimum of one (1) year Nurse Practitioner experience preferred.
- Previous Hospice experience preferred.
