What are the responsibilities and job description for the Case Manager position at GHR Human Services?
We are looking for a qualified and compassionate Foster Care Prevention Case Manager for the Philadelphia area. This position is responsible for providing care planning and intervention services to children who are at risk of going into foster care. The case manager works with the families for up to 90 days to get them resources to mitigate whatever the risk factors are, as well as obtaining resources to create a better home environment. Responsibilities:
- Prevention Case Managers collaborate with families to identity needs and achieve goals related to home safety, child well-being, improving school attendance, parenting skills, and overall self-sufficiency.
- This work involves conducting a thorough assessment of the strengths and needs of the families, developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs or services.
- Prevention Case Managers guide families through the process of enrolling in supportive programs using a strengths-based approach and respecting the family’s self-determination.
- This work also involves supporting families through advocacy and providing essential resources.
- Prevention Case Managers will specialize in serving families from one of two programs: Truancy: Case Managers are assigned to specific school sites and serve children with low school attendance who are at-risk for Truancy Regional Court.
- This involves collaborating with school personnel, frequent monitoring for monthly school and home visits, advocating for students in court, and working with families to eliminate barriers to academic success.
- Family Empowerment Center: Case Managers enhance the protective factors of families at-risk of child welfare involvement.
- This involves either monthly or twice-weekly home visits depending on tier, collaborating with a DHS social worker if there is an active case, and connecting families to resources that help improve overall well-being.
- The Prevention Case Manager applies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee-focused experience that is appreciative of the unique strengths, values and needs that each employee brings to the workplace; and by using data to inform and drive decision making.
- Attention is given to compliance, quality assurance, and best practices as they relate to employee experience, records management, and data integrity and usability.
Requirements:
- Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, etc.
- MUST HAVE DRIVER LICENSE AND CAR
- Some experience in social work and/or case management preferred but NOT REQUIRED
- Some experience with diverse populations (elderly, youth, etc.)
