A national post-acute & palliative care organization in the is seeking Nurse Practitioners to join their team due to continued growth! We are currently hiring a full-time and permanent Primary Care Nurse Practitioner in the Somerset Area.
This organization is experiencing major growth and is rapidly expanding in WV, PA, FL, IN & VA. They have a great reputation for working with the adult/geriatric patient population for over 20 years!
Responsibilities:
- As the Nurse Practitioner, you will be responsible for rounding and providing primary care in long-term care/assisted-living facilities in the area.
- Identifying short-term and long-term geriatric patient care issues that must be addressed and recommending courses of action.
- Assess patient health by interviewing patients, performing physical examinations, obtaining, updating, and studying medical histories
- Documents patient care services by charting in patient and department records
Schedule: Monday-Friday, Days – NO WEEKENDS*
Salary: $115,000 to $150,000

Qualifications:
- Current NP licensure & board-certification is required
- DEA license
- ICU, Hospital Medicine, Primary Care/Family Medicine and Adult-Geriatric or Palliative Care experience highly preferred
