Overview
Administers all resident admission processes from a medical condition standpoint to ensure that residents admitted may be properly served while continually communicating and coordinating with Case Managers, by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.
Responsibilities
- Processes referrals and requests for admission to the facility on a highly timely basis.
- Continually utilizes medical knowledge and experience as it relates to prospective resident situations and conditions when determining how best to serve those individuals based upon their medical conditions and needs.
- Undertakes all duties and activities in a manner aimed exclusively at resident satisfaction and health.
- Communicates with the Director of Nursing or designee regarding clinical facets of care for prospective residents.
- Facilitates communication between clinicians of the referring clinic and the facility to affect a smooth transition for prospective residents, both socially and medically.
- Makes all decisions based upon prospective resident clinical needs and the facility’s ability to deliver successfully upon those individual clinical needs.
- Maintains continually updated knowledge regarding facility and staff capabilities to meet individual resident needs when determining how best and whether it is possible to meet prospective resident needs.
- Organizes, revises and updates admissions systems while remaining fully coordinated with the facility management team and focusing upon resident, family and community needs.
- Interviews patient or representative to obtain and record all personal admitting information with full accuracy.
- Explains facility regulations such as visiting hours, behavioral requirements, emergency procedures, dining options, and safety requirements/procedures.
- Maintains effective ongoing communications with Case Managers regarding resident needs and medical conditions.
- Ensures that all duties are performed in a highly organized manner while contributing to facility efficiency and continually improving resident service levels.
- Ensures high capability and high productivity on the part of all directly supervised employees.
- Ensures that all necessary patient information is collected and utilized fully to ensure dignified and highly effective resident services.
- Enters patient admitting information into computer and routes printed copy to designated department.
- Obtains signed agreement from patient or representative to protect facility and other resident interests.
- Communicates planned admissions to all necessary departments.
- As necessary, performs bedside nursing duties.
- Complies fully with all HIPAA regulations and maintains all resident information highly confidentially.
- Assigns admitted residents to rooms and facility sections.
- Accurately compiles data for occupancy and census records.
- Coordinates with Unit Clerks regarding necessary follow-up actions, transportation, and changes to service levels.
- Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.
- Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.
- Performs other special projects and duties as assigned.
Qualifications
• Completion of a Licensed Practical Nurse program from an accredited college• Licensed Practical Nurse Licensure in AZ or qualified Compact License current and in good standing• IV certification completed• Current and continuing healthcare provider BLS certification• 1 year of recent experience within a residential long-term care facility• Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures• AZ Department of Public Safety Fingerprint Clearance Card current and in good standing required
