Role Description
This is a full-time remote role for a Social Worker. The Social Worker will be responsible for providing counseling, support, and care to individuals with serious illnesses, helping them manage their physical and emotional needs. The Social Worker will conduct assessments, develop and implement care plans, provide crisis intervention, coordinate resources, and advocate for clients. Additionally, the Social Worker will collaborate with interdisciplinary teams, provide education and support to clients and their families, and maintain accurate and timely documentation.
Qualifications
- Master's degree in Social Work
- Licensed Clinical Social Worker (LCSW) or eligible for licensure
- Experience in counseling individuals with serious illnesses and their families
- Knowledge of and experience with end-of-life care and bereavement support
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and remotely
- Compassionate and empathetic nature
- Ability to maintain confidentiality and uphold professional ethics
- Experience with electronic medical records (EMR) systems
