Job Overview: Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy. The Case Management Supervisor is responsible for overseeing the daily operations of the case management department, ensuring quality services are delivered to students, and maintaining compliance with applicable laws and regulations. This role involves providing leadership, direction, and coordination of the Case Management team.

Case Management Responsibilities and Duties:

· Oversight and supervision of the student intake and interview process interviews, assessing needs, and formulating individual employment plans.

· Establishing and maintaining rapport with students to enhance the overall student experience and well-being.

· Tracking and managing students from intake to testing, facilitating career development, and ensuring successful job placement.

· Designing, Implementing barrier reduction strategies to support students in overcoming obstacles.

· Proactively engaging students through face-to-face case management, ensuring a personalized approach.

· Collaborating with various stakeholders to provide necessary resources and services.

· Collaborating with recruiters, trainers, program administrators, industry managers etc., to ensure consistent, effective communication and resolution of student issues.

· Developing, coordinating, and securing Federal, State, and local benefits for students.

· Other duties as assigned by the Executive Director or Administrative Director.

Leadership and Supervision:

· Manage the performance of Case Management team.

· Provide leadership and direction to a team of case managers, ensuring the delivery of high-quality services to students.

· Supervise, coordinate, and monitor day-to-day operations of assigned Case Managers.

· Facilitating weekly interdisciplinary meetings to communicate progress and address obstacles.

· Assisting students in obtaining key documents, transportation, and necessary resources.

· Teaching essential life skills, including financial skills and budgeting.

· Advocating for students to ensure fair and consistent services and public benefits.

· Establish, and maintain a network or program resources for student’s needs (housing, childcare, etc.)

· Contributing to the development and implementation of a progressive engagement model.

· Staying up to date with local state education and employment laws.

Policies and Compliance:

· Developing and implementing policies and procedures to ensure that the training organization remains in compliance with employment laws and regulations.

· Conduct regular case reviews to ensure client needs are met and services are provided appropriately.

· Establish and maintain grant outcome reporting procedures as outlined with grant funding requirements

Intake and Case Assignment:

· Manage the intake process and assign cases to appropriate Case Managers based on workload and expertise.

· Conduct intake interviews and assessments as needed.

Collaboration and Communication:

· Collaborate with other departments and community organizations to ensure students have access to necessary services.

· Maintain constant communication between funding sources, case management team, and senior management.

Monitoring and Documentation:

· Monitor the department's budget, ensuring efficient and effective resource utilization.

· Maintain accurate and up-to-date records of student information, services provided, and outcomes achieved.

· Document student progress, barriers, and IEP in the Case Management System.

Client Support and Advocacy:

· Where applicable - Assess treatment needs, develop, monitor, and evaluate treatment plans and progress for students.

· Provide advocacy for needed services and entitlements.

Staff Development and Training:

· Participate in staff training and development to ensure case managers have the necessary skills and knowledge.

· Improve staff competence by providing educational resources and evaluating learning applications.

Reporting:

· Prepare reports by collecting, analyzing, and summarizing treatment data and trends.

· Complete grant and subsidy applications as necessary.

Requirements:

· Knowledge of practices and principles of social service program administration.

· Ability to lead, motivate, and evaluate staff.

· Ability to develop, implement, and enforce program policies and procedures.

· Excellent organizational, interpersonal, and communication skills.

· Ability to work with a diverse population.

· Ability to provide individual and group counseling.

· Ability to prepare accurate and complete records, reports, and statistics.

· Ability to meet attendance requirements.

Qualifications:

· Bachelor’s degree in Human resources, Social Work, Psychology or related field.

· Proven experience in case management, preferably in a training or education setting.

· Strong interpersonal and communication skills.

· Knowledge of community resources and services.

· Ability to engage and build rapport with diverse individuals.

· Detail-oriented with excellent organizational and documentation skills.

Pay: $60,000.00 - $85,000.00 per year
