Diabetes Management Partners, LLC d/b/a GlyCare contracts with hospitals to provide specialized glucose management teams to care for hospitalized patients with diabetes and blood sugar abnormalities. Our advanced practice providers apply clinically proven processes for improving glycemic parameters and economic outcomes for hospitalized patients with diabetes and hyperglycemia.
The American Diabetes Association recommends using dedicated inpatient diabetes management teams to improve glycemic parameters and economic outcomes in hospitalized patients with diabetes or hyperglycemia. Facilities partnered with GlyCare have seen reductions in the average length of stay, reductions in insulin-related errors and serious safety events, and improved average blood sugars across the facility's hospitalized population.
We are seeking Nurse Practitioners to become part of our team! You will focus on providing high-quality patient care as part of a specialized healthcare team in the hospital inpatient setting. Practitioners will go through extensive training to diagnose, treat, and manage diabetes in hospitals.
Openings are at Ascension Providence. Once hired, we would need to begin credentialing for an August start date.
RESPONSIBILITIES
- Implement patient management policies and procedures as per Company protocol
- Follow Company protocols to diagnose and treat diabetes in hospitals.
- Prescribe all necessary medications and treatments as protocol dictates
- Perform comprehensive physical examinations of patients
- Communicate with collaborating physician or specialist regarding patient care
QUALIFICATIONS
- To be considered, you must be certified as a nurse practitioner and or be a licensed nurse practitioner in the state of AL.

- Medicare provider number is preferred but not required
- Previous inpatient experience in nursing or other medical fields required
- A minimum one-year contract is required
- The ability to thrive in a fast-paced environment is required
- Must be detailed oriented
- Excellent written and verbal communication skills
- Ability to be efficient while managing your schedule without immediate supervision
- Ability to quickly learn hospital electronic medical records software
- Hospital privileges - C19 vaccine is required by the facility
THE DETAILS
- Practitioners will learn how to treat and manage adult hospitalized patients with diabetes and or hyperglycemia.
- Patients have clinical criteria for which a consult is automatically triggered.
- You are expected to see 20 to 28 patients per day (typically 28-32 per day when the census is higher). These visits focus only on blood sugar control and management.
- The start time is 7:30 am. We leave when the last patient is seen. Depending on the number of providers rounding with you, the day typically ends between 2:30 - 4:30 (7 to 10 hour day).
- PRN coverage is available and will be mandatory from time-to-time.
- Call responsibility is assigned on a rotating basis among all practitioners. Call can be taken from your home via the phone and does not require you to come back to the facility. Typically call would be 1 to 2 times a month.
- Additional monthly bonuses are paid on Productivity, Volume, and Quality.
- GlyCare pays 100% of the employee portion of the health insurance. You may choose to buy-up health plans as well. Health insurance via BlueCross

- An iPhone is required. A phone stipend of $600 is provided.
OPEN SHIFTS
Location: Ascension Providence 810 Saint Vincents 6801 Airport Blvd, Mobile, AL 36608
- Wed/Thursday/Friday and every other weekend. This equates to 16 days per month on average. Base salary is $90,000.
