Social Services will be responsible for the development and implementation of the social services component.

The Social Worker will provide but not limited to:

-interviewing the members and caregivers to obtain a social assessment and evaluation of needs and problems

-preparing the social work portion of the patient plan of care on the basis of the assessment and providing services

-providing or arranging for individual, family and group counseling in reference to psychological, social, financial, legal, vocational, and educational needs of the recipients

-assisting with obtaining concrete services such as housing, shopping, clothing etc

-referring to and/or developing support groups and educational programs for caregivers and recipients

-participating in all members case references such as pre-admissions and post admissions, problem-oriented case

-reassessing the members when necessary and revising the social work portion of the clients plan of care

-documenting assessments, evaluations, treatment plans, clinical notes etc in the patients medical records

Job Types: Full-time, Part-time

Pay: $20.00 - $28.00 per hour
