We are looking for an experienced Memory Care Coordinator to join our team.
As the Memory Care Coordinator, you will be responsible for managing and coordinating care services for individuals with Dementia. You will oversee the person centered care and activities to support a vibrant lifestyle. This is a rewarding opportunity to make a positive impact on the lives of our residents!
Responsibilities:
- Manage and oversee the residents as well as the caregivers.
- Coordinate and schedule appointments, activities, and therapies for residents
- Ensure resident rights are respected and upheld at all times
- Develop and implement individualized care plans in collaboration with healthcare professionals
- Monitor and assess the physical, mental, and emotional well-being of residents
- Provide leadership and guidance to a team of caregivers
- Conduct regular meetings with staff to ensure effective communication and coordination of care
- Assist with budgeting and financial management related to care services
- Collaborate with external healthcare providers, including nurses and home care agencies
-This position reports to the Wellness Director
Skills:
- Strong knowledge of resident rights and regulations related to senior care.
- Excellent organizational and time management skills
- Leadership abilities to guide and motivate a team of caregivers
- Experience in working with dementia or memory care needs is preferred
- Familiarity with care services and nurse coordination is an asset
