What are the responsibilities and job description for the Case Manager position at Good News Home Inc.?
The primary function of this position is to assess the needs of the client, arrange and coordinate the delivery of essential goods and services provided by other resources. This position works directly with the client and the client’s team to develop and be prepared to implement an immediate and long-term discharge plan. This position also advocates on the client’s behalf with the necessary legal, financial, environmental, and religious institutions. It will be the Case Manager’s responsibility to ensure that the services provided are timely, in keeping with the treatment plan developed and support the holistic approach to recovery of this program.
Duties and Responsibilities
- Compile initial assessments and maintain ongoing assessments/evaluations of each client's family, support, legal, financial, educational, and vocational needs.
- Manage the biosocial/legal aspects of each client.
- Maintain complete and accurate records of all activities related to and in support of the client’s recovery process.
- Schedule and transport the client to court hearings or other legally mandated appearances when necessary.
- Complete evaluations and meet scheduled intervals for ongoing program compliance evaluations.
- Interact with and be available to all women in the program.
- Maintain timely communication with staff regarding client issues.
- Provide appropriate interventions in crisis situations.
- Read logs and integrate information about residents into interactions with clients as deemed necessary and helpful.
- Document attendance at all job-related training and seminars by submitting a certificate of completion for inclusion in the employee file.
- Attend staff meetings.
Requirements and QualificatioCADC certification is required. MSW degree is helpful.
- Management and organization skills are necessary.
- Experience working with women in a group home environment is both helpful and critical.
- Knowledge of the dynamics and interdependency of addiction and spiritual void.
- Assessment/evaluation skills.
- Ability to communicate clearly and effectively with staff, clients and the public.
- Valid driver’s license.
- Self-motivated and requiring minimal supervision.
