Community Health Worker
The Community Health Worker (CHW) will be responsible for helping patients navigate the healthcare environment. They serve as a frontline health worker who has as an unusually close understanding of the community served. The CHW assist with accessing community services and other resources; connecting patients to care; eliminating barriers and advocating for the adopting of healthy behaviors.
Essential Duties and Responsibilities:
1.
Assist the at-risk patient population by serving as a liaison between the community, medical, and social services.
2.
Assist at-risk patients to connect with healthcare providers, navigate the healthcare system, and access the resources they need for daily living (ex: food, supplies, housing).
3.
Provide outreach, education, social support and serve as advocates for patients with chronic disease.
4.
Establish trusting relationships with at risk patients and their families while providing general support and encouragement.
5.
Work collaboratively with other clinical personnel assigned to the same patient.
6.
Refer the at-risk patient and family to available community resources.
7.
Follow up with patients via phone call, and visits to other settings where patients can be found.
8.
Work closely with health care providers to help ensure that patients have comprehensive and coordinated care plans.
9.
Consistent communication with Coordinator of Clinical Quality and Compliance to evaluate patient/family status, ensuring that provided information, and EMR clearly describe progress.
10.
Provides support and advocacy during clinic visits or when necessary to assure patient’s medical needs and referrals are being conveyed.
11.
Coordinates and monitors services, including comprehensive tracking of patient’s compliance in relation to care plan objectives.
12.
Establish professional working relationships with patients, visitors, staff, and providers effectively communicating Goshen Medical Center’s mission.
13.
Establish therapeutic relationship with patients from initial identification through closure.
14.
Maintain accurate knowledge of available community resources.
15.
Assist with care gap identification and closure as directed by Coordinator of Clinical Quality and Compliance.
16.
Other duties as assigned.
Required Qualifications: Education/Experience
1.
High school diploma or equivalent.
2.
Experience working in a multicultural setting.
3.
Basic computer skills.
4.
Ability to develop and maintain positive relationships with staff, community resources, and other organizations.
5.
Understanding of and sensitivity to the at-risk community.
6.
Good communication skills, such as listening, speaking, and writing.
7.
Ability and willingness to provide emotional support, encouragement, and motivation to patients.
8.
Ability to work independently and be a self-starter.
9.

1.
Certified nursing assistant or certified/registered medical assistant.
2.
Recent employment in healthcare environment: 2 years
3.
Oral and written fluency in English and Spanish.
