SUMMARY:
The Discharge Care Manager must have basic knowledge of mental health and addiction recovery processes, services, and resources. The Care Manager promotes quality and value of care by engaging and educating patients to manage their health and decrease readmission rates post follow-up. The care manager performs comprehensive evaluations of patients at care transitions, monitors readmission rates, and communicates with patients who have identified care needs.
ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
- Conduct patient interviews (by-phone) and complete basic assessments regularly.
- Identify and contact high-risk patients as necessary for follow-up needs
- Participate in patient outreach and review of records to identify and close gaps in quality measures
- Support defining and improving readmission rates at 30, 60 and 90 days for all post-discharge patients
- Support defining and decreasing post discharge Emergency department visits, Inpatient behavioral health visits and withdrawal episodes.
- Support defining and decreasing post discharge overdoses for 90 days post follow-up
- Establish connections and utilize community resources to help patients develop effective recovery support systems.
- Work with patients to develop resources to help facilitate self-management of care and services.
- Participate in staff supervision and in-service training programs.
- Participate in team meetings to review patient progress.
- Maintain patient records in compliance with existing Gosnold standards.
- Perform all other duties as assigned.
- Works with Director of Marketing on Alumni programs
MINIMUM KNOWLEDGE, Experience, AND SKILLS REQUIRED:
The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job.
- Education: High School Degree.
- Experience: Two (2) years of experience in mental health and addiction.
- Skills: Strong interpersonal skills, strong organizational skills, self-directed, and proficient with web-based electronic health records.
- Certificates, Licenses, and Registrations: Valid clean Driver’s License.
