We are partnered with a highly regarded care group in the area that is adding a Care Coordinator to their team. This role includes a $5K sign on bonus and loan forgiveness with a flexible hybrid schedule .

Qualifications :

  • Master's degree in mental health, social services, or a related field
  • Certification as a qualified mental health professional (QMHP) through the Mental Health and Addiction Certification Board of Oregon
  • Valid Oregon driver's license
  • Full vaccination against COVID-19

Responsibilities :

  • Provide assessment and treatment planning services to individuals with a serious mental illness.
  • Assist clients as needed with case management needs and crisis intervention services.
  • Monitor and evaluate clients' progress towards specific goals around housing, employment, symptom management, and harm reduction.
