You make your own schedule for visits that work for you and the patients you are assigned to. This could be a side job for extra income. There are opportunities for full-time.
Home Health Aides with Grace and Glory Hospice visit and provide care to patients in their homes. Each patient has a different Plan of Care managed by the RN Case Manager and Medical Director. When you are assigned to patients you will be able to plan their HHA visits around your schedule as long as the frequencies of visits in the Plan of Care are being met. This is a great second job for someone looking for extra work they can do around their other job and family schedule.
This is a PER DIEM position. Visit Rate is $40.00 per visit. Hourly is $30.00 an hour for training/in-services and meetings. Plus 65.5 cents a mile to and from your visits.
Responsibilities and Duties
The Hospice Home Health Aide is responsible for providing personal care under the direction of a registered nurse.
- Perform Personal Care for Patients (bathing,dressing,toileting,feeding, etc) according to the Plan of Care
- Document care provided according to agency policy through our online system HospiceMD
- Report any deviations from the Plan of Care to the Case Manager (or designee if not available).
- Use durable medical equipment.
- Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
- Provide proper care and observation of the patient's skin to prevent breakdown of tissue over bony prominence.
- Reports any changes/areas of concern to Case Manager.
- Provide respite for patient's family/caregiver when on-site, as appropriate.
- Participate in QAPI activities.
- Participate in IDG meetings, as appropriate.
- NEVER dispense or administer medications.
Job Conditions
- The ability to access patients' homes which may not be routinely wheelchair accessible is required.
- Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
- Physical activities will include, walking, sitting, stooping, standing and minimal to maximum lifting of patients and the turning of patients.
Equipment Operation
- Utilization of calculator, telephone, copy machine, thermometer, blood pressure cuff, other basic office and medical equipment.
Requirements
- Home Health Aide Certification
- CPR Certification
- Physical Form signed by Doctor - (we provide this form)
- Recent TB Test and Titers (checks for previous vaccinations) or Immunization records
- 2 forms of ID (driver's license, social security card, passport, birth certificate)
