Job Information:
- Grand Traverse Pavilions is seeking a Case Manager.
- 40 hours a week with flexible scheduling.
- This position starts at $42.00/hour
- Must be a Registered Nurse.
- Must be able to pass a criminal background check and drug screen.
Tasks:
- New admission paperwork completion and initial nurse assessment. Completion of Level of Care determinations on admission.
- Discharge Follow-up Calls and Data Collection/Quality Assurance.
- Monitor OBRA services and follow through with recommendations. Organize and complete OBRA forms monthly.
- Facilitate safe discharge by coordinating all appropriate procedures working in partnership with doctors, nurses, therapists, and or outside agencies, i.e. Hospice team, ensuring that all plans are in place for time of discharge.
- Attends meetings to discuss the admissions and discharges, convey information to the relevant teams to improve upon processes.
