The Clinical Documentation Specialist (CDS) provides timely concurrent review of clinical documentation in the medical records by querying the medical staff and other caregivers as necessary via written/verbal communication to obtain accurate and complete physician/staff documentation that supports the severity of patient illness, risk of mortality, and the quality of care provided. This is done by performing a thorough chart review to identify co-morbidities and complications and document appropriately on departmental worksheets. There is great importance to make an effort to capture all potential secondary diagnosis, for appropriate DRG assignment for profiling purposes.Rotational remote work eligible.
Education and Experience:Associate degree in Nursing required. Preferred candidate will have 5-7 years or more Acute Care Clinical Documentation experience as Clinical Documentation Specialist as well as 5 plus years of Acute Care Hospital experience in Med/Surg, Tele or ICU. BSN and/or Certified Clinical Documentation Specialist (CCDS)/Clinical Documentation Integrity Practitioner (CDIP) credential with coding experience preferred.
