Hospice Clinical Resource Nurse (Registered Nurse) Position located at Granite VNA Concord Office.
Current opening: Monday - Friday 8 a.m. - 5 p.m.
The Hospice Clinical Resource Nurse – provides telephone triage calls from – hospice patients and families, skilled and assisted living nursing facility staff, physicians and their office staff, pharmacy, durable medical equipment providers and other types of calls related to active and pending hospice patients. Duties will include assisting with order entry and referral review for Hospice House.
Essential Functions:
- Clinically triages phone calls from patients/families/caregiver by providing skilled and palliative care assessment, problem solving and instruction along with assisting to coordinate home visits as indicated for patients who call the agency seeking guidance
- Assists with entry of medications and orders for Hospice House patients.
- Provides guidance including pain management and symptom control; conducts assessments and evaluations and provides education and support care to patients and caregivers in collaboration with the nurse case manager.
- Coordination of patient care with the physician office and enters orders as indicated. Communicates outcome with clinical team.
- Receives and contacts by phone/fax/email any patient physician (designee) as indicated by the change in the patient’s condition, treatment and/or status.
- Accepts and completes all necessary documentation related to accepting medications and/or Durable Medical Equipment for Hospice patients.
- Completes comprehensive documentation about the calls received/initiated in the patient’s medical record using the SBAR guide.
- Assists daily with clinical triage of community visits and admissions for hospice program.
- Supports scheduling with determining priority of assigning open visits.
- Assists with chart review and monitoring of quality activities as requested.
- Administers medications and treatments as prescribed by the physician.
- Assists with review of referrals for Hospice House as requested to ensure timely determinations of appropriateness for Hospice House bed.
Other job functions:
- Acts as a clinical resource to the Hospice clinical team
- Demonstrates flexibility in the role by accepting other duties as assigned for which qualified
Requirements
- Graduate of an accredited school of nursing, accompanied by current New Hampshire RN license, or compact state license through the Nurse Licensure Compact (NLC).
- One-two years of hospice experience.
- One-two years of telephone triage experience preferred.
- Certification in hospice nursing (CHPN) preferred.
