We are presently seeking Community Health Workers/Care Coordinators in our Medical Division.
The Community Health Worker/Care Coordinator position will primarily work with specific target populations in order to educate patients on disease states, engage patients in care, and coordinate care plan objectives for the purposes of improving patient care and health outcomes. Coordinates with clinical practice teams through all phases of the care continuum. Position will work with both internal and external healthcare entities to coordinate health services with individuals, families, and the community. Expedites a common goal throughout the continuum of care to achieve coordinated high quality care that is patient centered.
The Community Health Worker/Care Coordinator position requires Bachelor's Degree in Public Health, Social Work, LPN, or minimum of 2 years relative experience working within a Community Health setting; Working knowledge of HMO's, PPO's, Medicare/Medicaid and other insurance plans; A minimum of 2 years of experience working with the Medicaid/Medicare population; Knowledge of culture and values of community; Familiarity with the resources available in the community; Experience working in a field-based position.
