Position Title: Service Coordinator
Assignment Level: Classified - 6
Direct Supervisor: Community Resource Supervisor
Qualifications: High School diploma or equivalent. Parent of a child with special needs or knowledge of early childhood development and family dynamics.Ability to communicate well both orally and in writing. Excellent organizational skills. Driver’s license and personal vehicle required.Ability to make connections with students, families, schools, and agencies.
General Summary: To partner with families to meet their unique needs within an integrated system of early intervention services and within natural communities of support.
Essential Functions & Responsibilities:
Coordinating Early ACCESS and community services across agency lines and facilitate the timely delivery of available services
Serve as the single point of contact in helping parents to obtain the services and assistance they need, including medicaid and waiver assistance.
Assist parents of eligible children in gaining access to early intervention services and other services identified in the IFSP(Individualized Family Service Plan).
Coordination of the provision of early intervention services and other services (such as medical services for other than diagnostic and evaluation purposes) that the child needs or is being provided.
Continuously seeking the appropriate service(s) and situations necessary to benefit the development of each child being served for the duration of the child’s eligibility.
Help to identify family’s needs and concerns. Facilitate and participate in the development, review, and evaluation of IFSP.
Inform and assist families in identifying advocacy services, family resources, informal networks and available service providers.
Coordinate with medical and health providers; and ensure all necessary records are obtained.
Facilitate the development of a transition plan to preschool services, if appropriate.
Participate actively in weekly Team meetings and ensure the Individual Family Staffing Report is updated before team meetings occur.
Follow the Agency-prescribed intake process.
Maintain an educational file.
Complete and submit all IFSP paperwork in a timely manner. SeekICD-10 diagnostic codes for medicaid billing, bill medicaid monthly, send Thank You notes to referral sources, and upload relevant documentation into the IFSP system.
Ensure all initial, periodic, and annual IFSP meetings occur within the designated time frames.
Complete ECO Maps with all new referrals to establish both formal and informal networks of support
Schedule meetings with service providers, families.
Perform other duties as assigned.
The position also serving the AHSTW, Boyer Valley, IKM Manning, Woodbine, and Lamoni areas.
Physical Requirements:
Regular attendance is required.
Ability to travel within assigned schools
Ability to work flexible hours, including minimal evenings and weekends
Requires good eye/hand/finger coordination to keyboard and to operate office machines. Ability to communicate effectively orally and in writing. Good concentration and visual skills required. Normal, routine levels of activity related to bending, carrying, hearing, lifting, reaching, sitting, standing, vision and walking and may also involve above-average levels of activity at times that can’t always be anticipated.
Provisions of Employment:
The period of employment shall be for the number of days as specified by the adopted calendar with an understanding that additional days may be negotiated as determined by need.
Conditions of Employment:
The Service Coordinator shall be under contract with the Green Hills AEA Board of Directors and shall abide within and benefit from all directives, procedures, and policies developed by Administration.
