MAIN FUNCTION: Provides medical social work services for the hospital population and community, and advocacy and assistance to other health care consumers and community groups as requested. Provides support to the Emergency Services Department in absence of the Care Coordinator. Assists with end-of-life discussions and high risk readmission assessments in collaboration with Case Management.
Essential Responsibilities & Duties
Screens 100% of high-risk patients admitted to the hospital within one working day of admission in order to evaluate their needs for further intervention and follow-up.
Utilizes a variety of casefinding methods to assure early identification of patients in need of social work intervention.
Identifies and utilizes community services and financial resources to meet patient/family needs.
Performs psychosocial assessments of patients/families and develops plans to assist them with problem resolution.
Participates in evaluation of the quality of services provided.
Demonstrates an awareness of and adherence to all institutional, regulatory, legal and professional guidelines governing the provision of mental health services.
EDUCATION: M.S. W. degree from an accredited school with specialization in medical social work.
EXPERIENCE: Two years relevant post-graduate clinical experience preferred; L.C.S.W. required
