JOB DESCRIPTION: Case Manager
Reports to: LCCA
Overview:
The Case Manager is responsible for developing a comprehensive service plan that meets the individualized needs of the residents in accordance with Agency policies, and procedures, licensing standards and other federal, state, and local regulatory requirements.
Responsibilities:
Work Standards
- Maintain an active caseload and provide case management services that meet the needs of the residents and is in accordance with Agency policies, licensing and funding standards, and other federal, state, and local regulatory requirements
- Utilize clinical knowledge and skills, conduct assessments to determine resident needs, including but not limited to behavioral interventions, basic needs, and possible barriers to achieving individual service plan goals; identify specialized counseling and case management service needs, mental and emotional health concerns, and family reunification issues
- Shares in on call schedule
- Other duties and special projects as assigned
- Communication o Complete, update and maintain accurate records of all resident interactions, contacts, behavior issues, and resident progress; collect and compile relevant case history information; ensure all case files include the required documentation and required forms; maintain case files in accordance with Agency policies, licensing standards and other federal, state, and local regulatory requirements; ensure all case files are always audit ready o Ensure the privacy of all resident information and case files; always comply with
HIPPA/HIPAA guidelines o Complete and submit all progress and case notes, reviews, and reports in a timely manner
Adaptability
- Provide crisis intervention and intense case management services based on resident need; report serious or significant incidents to the appropriate parties in a timely manner; complete the necessary paperwork and provide the required documentation of the crisis or situation
- May be required to complete driving duties as driver and/or accompanying during transport. Subject to being designated as an authorized driver
- Teamwork o Work in collaboration with the interdisciplinary team to develop and implement specific activities in the areas of social skill acquisition, health maintenance and appropriate nutritional needs o Work closely with the clinical and program teams to assess social, educational, and emotional needs of the resident and devise a service plan or behavior plans to meet those needs; develop, write, and update service plans, reports and correspondence o Work closely with the team and other service providers to ensure residents are receiving the quality of care and case management services needed and the program services are meeting its established goals and objectives
Building Relationships
- Serve as an advocate or liaison between the residents and the court system; accompany residents to court appearances, as needed
- Respond to resident needs and concerns with compassion and empathy in a timely manner
- Work closely with case management staff, interdisciplinary teams, and other program service providers to ensure residents are receiving the full scope of available program services and resources to meet youth’s Plan of Service (YPOS)
- Work as a team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with
Agency policies and practices
- Actively participate in exhibiting cultural awareness and sensitivity when interacting with youth, parent(s)/guardian(s), staff, and the community
Qualifications:
Minimum Qualifications
- Bachelor’s degree in Social Work, Counseling, Psychology, or closely related field
- Previous case management experience
- Must have valid TX driver license and auto insurance to be designated as an authorized driver
- Knowledge of psychological theory, methods, and ethics
- Strong working knowledge of motivational interviewing
- Strong writing skills with the ability to document resident care and progress properly and accurately
- Ability to follow instructions and work independently and within a group and with minimum supervision
- Strong organizational and time management skills with the ability to meet deadlines
- The ability to relate positively to young people with serious behavioral and emotional disturbances
- Strong and effective communication (verbal) skills with the ability to clearly communicate thoughts and provide instruction
- Strong interpersonal skills with the ability to apply compassion and empathy, while dealing with stressful and difficult situations
- Strong interpersonal and people skills with the ability to work and interact with diverse groups of people
- Proficient in Microsoft Word, Excel, and Outlook
- Preferred Qualifications o Master’s degree in Social Work, Counseling, Psychology, or closely related field o LMSW, LCSW, LMFT, or LPC o Experience working in a Residential Treatment Center or Behavioral Health
