Job Overview:
Seeking a Nurse Practitioner for face-to-face home visits with hospice patients, ensuring high-quality care.

Responsibilities:

  • Conduct comprehensive patient assessments during in-home visits
  • Develop and implement personalized patient care plans
  • Perform diagnostic tests and analyze results
  • Administer medications and treatments in-person
  • Collaborate with healthcare professionals for optimal patient outcomes
  • Educate patients and families on health management during face-to-face interactions

Qualifications:

  • Valid Nurse Practitioner license
  • Experience in specimen collection
  • Familiarity with discharge planning and ICD-9 coding
  • Proficiency in aseptic techniques
  • Experience working with developmental disabilities
  • Competence in eClinicalWorks software
  • Ability to perform Botulinum toxin treatments

Preferred Skills:

  • Experience with inhale systems
  • Familiarity with eClinicalWorks

Job Types: Full-time, Part-time, Per diem

Pay: $80.00 - $120.00 per hour
