What are the responsibilities and job description for the Jail In-Reach Coordinator position at Gulf Bend Center?

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are: psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

Preferred: At least six months experience in providing mental health services to adults, children and/or adolescents. Work experience working within jail setting. Knowledge of DSM-IV/V disorders useful.

DEGREE AND/OR CERTIFICATION REQUIRED: BA/BS in field specified above.

REPORTS TO: Director of Behavioral Health Services

LEVEL OF SUPERVISION REQUIRED: General

JOB PURPOSE: This position will be an integral part of coordinating and providing forensic services with the focus of restoration for persons who have been determined "Incompetent to Stand Trial" under the Texas Code of Criminal Procedure, Chapter 46B, and who are awaiting admission to a state hospital. This position will collaborate with local forensic teams to support active forensic waitlist monitoring of people waiting in jail for court-ordered inpatient competency restoration services. They will provide direct services to the person as well as link to additional needed resources to include psychiatric services, medical services, and legal education. Goals include improving health outcomes for people waiting for inpatient competency restoration programs. This position will work closely with several partners to include jail staff and administration, legal counsel, JP/local judges, and the state (HHSC) Forensic In Reach team. This position will also provide enhanced continuity of care following a person's restoration to competency and return to jail to prevent clinical decompensation and unnecessary rehospitalization. They will also participate in comprehensive strategic planning and implementation process to achieve sustainability for the jail-in reach program. Will work closely with the criminal justice system and acts as liaison to work in cooperation with community organizations including, but not limited to law enforcement and other community providers. Will require travel within the 7 counties. Meets multiple deadlines and reporting requirements. Work is performed under the general supervision of the Director of Behavioral Health Services with considerable latitude for the exercise of discretion and independent judgment.

ESSENTIAL FUNCTIONS and DUTIES:

1. Monitors 46B commitments while in jail through waitlist reports weekly.

2. Responds to new referrals and/or notifications from the report(s) within 2 business days.

3. Identifies if inmate is admitted to GBC per clinical system.

4. Assesses inmate to identify current needs and develop plan of care.

5. Links, coordinates, and organizes referrals services as needed to be provided. Acts as a liaison to additional services.

6. Provides direct service to include psychoeducation, medication training and support, and skills training to enhance and restore competency.

7. Provides follow up as needed while detained in jail.

8. Communicates and collaborates with the State Hospital Wait List In-Reach Team

9. Utilizes tracking spreadsheet for caseload and status purposes.

10. Effectively communicates with all team members involved with each person/inmate.

11. Provides follow up in the community as needed when inmates are released.

12. Coordinates with jail staff for visits and services for inmates.

13. Submits all documentation of services within two business days.

14. Participates in team meetings and has data prepared as needed.

15. Meets with community partners to build relationships and educate on jail in reach services.

16. Coordinates and links person/inmate to community supports and services to meet their needs.

17. Answers inquiries by providing accurate and appropriate responses in writing, via e-mail, or using telephonic means.

18. Must maintain regular and reliable attendance.

19. Demonstrates efficient time management as evidenced by meeting all job expectations.

20. Uses the trauma informed care approach when providing services and working with others.

Trauma-informed:

A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include: safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

Work site and environmental conditions that apply to this job: Travel by car 50% of time, outside, inside, working closely with others, working alone.

OTHER REQUIREMENTS OR CONDITIONS:

1. Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.

2. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.

3. Good driving record as defined by Gulf Bend Center.

4. Must complete all CORE training requirements prior to the due date.

5. Must have computer experience to include: Microsoft Word, Excel, and Outlook. Must have good organizational skills to achieve multiple deadlines. Ability to type 40 wpm.

6. Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
