POSITION TITLE: Community Resources Specialist

GENERAL PURPOSE:
Specialist will offer the community resources, referrals and service coordination for grant funded programs such as Regional Care Coordination Organization, Healthy Communities, Handicapped
Children’s Program and other care coordination services. Case Management for enrolled members will be offered and coordinated when appropriate. It is the intent for the specialist to help consumers navigate the systems of services currently being provided in Gunnison County and to assist access to care throughout the state.
SUPERVISION RECEIVED:
The specialist will be supervised by the Administration Services Supervisor or other appropriate supervisor as determined by the Executive Director.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Assure and provide assessment of the individual needs and service coordination for clients of specific programs. Provide information and referrals about local, regional and national resources. Specialist may provide assistance with registrations, etc. in order to facilitate a coordinated, comprehensive response to identified needs.
2. Work with various community agencies to facilitate coordination of services.
3. Provide all required reporting to grantors as per contract obligations.
4. Attend meetings, conferences etc. as required by grantor.
5. Compile/provide statistical information for community planning and grant writing.
6. Respond to requests from community agencies/service providers to provide assessment and coordination of after care and/or follow up.
7. Advertise and outreach in the community as is appropriate for specific grants.
8. Maintain and/or develop policies/procedures/forms as needed.
9. Responsible for demonstrating competencies in areas of Emergency Readiness as deemed necessary by Executive Director and/or Emergency Services Staff. This includes participation in department wide exercises or activities related to Emergency Response.
10. Other duties as assigned.
DESIRED MINIMUM QUALIFICATIONS
Education and Experience:
- Bachelor’s degree in one of the human behavioral science fields.
- Ability to organize, set priorities, work independently and work well with community partners.
Necessary Knowledge, Skills and Abilities:
- Knowledge of casework principles
- Knowledge of medical terminology
- Knowledge and skills in interviewing techniques
- Knowledge of community resources
- Ability to clearly and concisely express oneself both verbally and in written format
Special Requirements:
Completion of ICS 100 and 700 within 12 months of hire. Valid Colorado Drivers License.
TOOLS AND EQUIPMENT
Use of desktop or laptop computer and printer, FAX machine, copier, scanner, other basic office equipment and use of automobile.
PHYSICAL DEMANDS
Independently mobile, type, use hands and fingers, reach with hands and arms, talk, listen, hear, read see, lift, climb in and out of a vehicle, drive a vehicle, climb stairs, and able to lift 25 lbs.

WORK ENVIRONMENT
Normally, indoors in a controlled office environment. Frequently, home visits or meetings off site are necessary. Moderate noise. Many DHHS clients are children. May come in contact with infectious waste during course of duties.
ELEMENTS OF CONTACT
Personal contact with any individual of the public is normally a single occurrence. Hand to hand exchanges of documents, money, personal property and other items may be required. Face to face verbal communications between the member and an individual is required but is mainly singular. The member’s anticipated contact with blood, body fluids or other medical hazards are rare. The member should have adequate time to take precautionary and protective measures.
