Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), and our Gurwin Certified and Licensed Home Care Services Agencies.
Our innovative facility and growing services offer opportunities to advance your career. We are currently seeking a full-time Medicaid Unit Assistant in our Finance Department. The salary range is $60,000-$65,000 based on experience.
EDUCATION AND EXPERIENCE REQUIREMENTS:
High School Diploma - Minimum 2 years SNF, DSS or related experience.
REQUIREMENTS INCLUDE:
· Conduct Pre-admission and Financial Interviews with prospective family members according to the Finance
· Department Policies and Procedures, collect documentation required for Medicaid application, review facility
· financial policies with prospective resident family members, have required financial agreements signed, explain
· Medicare and Medicaid regulations that apply to a prospective resident family member.
· Provide family with a summary of information needed to complete Chronic Care Medicaid application.
· Follow up on Rollovers and Long Term Direct Admits and arrange interviews with families.
· Supply Pharmacy with copy of prescription plan ID cards.
· Advise supervisor, upon request, regarding status of interviews and approvals.
· Update and maintain Recertification records from the monthly Roster review, and as scheduled by the related
· DSS office.
· Process recertifications for appropriate county and follow-up with residents and families in a timely and
· professional manner.
· Review all new budgets to ensure we received correct Recertification, Conversion, and Provider Change
· information. Process Conversions and Provider Change requests with knowledge and efficiency.
· Process Medicaid Applications as assigned by CFO or Director of Finance by meeting with residents, families,
· attorney’s and respective Medicaid office in a timely and professional manner in order to obtain all necessary
· documentation.
· Process revised Pick-Up dates when needed, and follow-up with Attorney’s, Case Workers, and Elder Care
· Agencies to notify them of any changes on a timely basis.
· Process new, corrected and revised Budgets in a timely and accurate manner with related parties (ie: Attorney
· Offices, EMEDNY for CIN# verification, Nami Coordinator, etc.).
· All other Finance duties as required.
